All holders of operating licenses or construction permits for nuclear powerreactors.

Purpose

This information notice is intended to alert addressees to a recent revisionto the NRC Operations Center's "Event Notification Worksheet" and delineatesthe event notification Information that is sought by the NRC Operations Officers.However, suggestions contained in this information notice do not constitute NRCrequirements; therefore, no specific action or written response is required.Discussion:The NRC "Event Notification Worksheet," NRC Form 361, has been revised toassist the NRC Headquarters Operations Officers in obtaining adequate infor-mation for evaluation of significant events reported to the NRC OperationsCenter. The new forms more accurately reflect the event classifications andthe 10 CFR 50.72 categories that must be reported. A copy of the new work-sheet is enclosed for your reference. NRC Form 361 can be ordered from theNRC Information and Records Management Branch at (301) 492-8138.The blocks for the "Cause of Failure," "System," and "Component" have beendeleted, and this information is now included under the description sectionwhere it can be more fully described. As in the past, the description is toprovide adequate detail so that the Operations Officer will be able to under-stand the event and its significance. It is recognized that all the informationregarding an event may not be available at the time of notification. However,reports are to be as clear and complete as possible within the time allowed bythe inmediate notification requirements. If significant information not includedin the original report is identified, original information requires correction,or the classification of an event is changed, this information can be reportedin an update notification to the Operations Center when it becomes available.The back of the form, which is used to collect information on radiologicalreleases and/or reactor coolant system (RCS) or steam generator (SG) tubeleaks, has been revised to provide a simpler and more structured method ofobtaining and evaluating information pertinent to these types of events.8912190332I bIw IN 89-89December 26, 1989
