7 APPENDIX A NOTICE OF VIOLATION Baltimore Gas and Electric Company Docket / License Nos.:

50-317; DPR-53 Calvert Cliffs Nuclear Power Plant 50-318; DPR-69 During an NRC inspection conducted from November 15, 1989 through December 31, 1989, and in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C), the following violations were identified.

i 10 CFR 50, Appendix B, Criterion XIII, " Handling, Storage and Shipping," and the Calvert Cliffs Quality Assurance Plan state that measures shall be estab-lished to control the storage and preservation of equipment in accordance with work and inspection instructions to prevent damage or deterioration.

Chemistry Procedure (CP) 217, " Specifications and Surveillance - Steam Generators," implements the above requirements and specifies that, during cold shutdown and refueling modes, steam generator secondary side water pH should be maintained between 9.8 and 10.5 and hydrazine concentration should be maintained between 70 and 200 ppm. CP-217 also states that if a parameter degrades beyond its specifications, it should be returned to within specification to prevent corrosion to the steam generators.

Contrary to the above, between May 1,1989 and December 15,1989, the pH levels and hydrazine concentrations for steam generator Nos.11,12, and 22 were not maintained within the required specifications.

This is a Severity Level IV Violation (Supplement I).

Title 10 CFR 50, Appendix B, Criterion V requires that activities affec-ting quality shall be prescribed by documented instructions, procedures or drawings, of a type appropriate to the circumstances and shall be accom-plished in accordance with these instructions, procedures or drawings.

l The applicable electrical cable installation and separation requirements are implemented in " Design and Construction Standards for Cable and Race-way" (E 406).

E 406 requires that each of six cable separation groups be i

physically separated from one another horizontally (three feet minimum) l and vertically (five feet minimum) or by an intervening barrier.

Cable tray ZAIAL11 (separation group 1) was routed within three feet horizontally of cable tray B1AE40 (separation group 2),

Cable tray ZA1A35 (separation group 1) was routed within three feet horizontally of cable tray B1AE40 (separation group 2).

C.

Cable tray ZA1AL10 (separation group 1) did not have adequate separa-tion from cable trays BIAE41 and BIAL 14 (separation group 2) due to a damaged barrier.

D.

Cables in tray ZBIAE76 (separation group 2) were also routed in tray ZFIAE77 (separation group 3).

E.

Cables in tray ZA1AE70 (separation group 1) were also routed in tray ZGIAE72 (separation group 3).

This is a Severity Level IV Violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Baltimore Gas and Electric Company i

is hereby required to submit to this office within thirty days of the letter which transmitted this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
