NOTICE OF VIOLATION (VERMONT YANKEE INSPECTION 93-14)

This refers to the safety inspection conducted by Messrs. H. Eichenholz.and P. Harns on

i

'

June 27 - August 7,1993 at the Vermont Yankee Nuclear Power Station, Vernon, Vermont.

The preliminary results were discussed with Mr. R. Wanczyk at the conclusion of the inspection.

The inspection focused on issues important to public health and safety, and consisted of

performance observations of the conduct of operations and maintenance, and independent

evaluation of safety system status and quality records.

Our observations this period were that the plant was operated safely, and that your staff

responded well to a July 30th lightning strike. However, we are concerned that management

'

of your maintenance contractor diminished this period, as evidenced by their creation of an

unrecognized temporary modification condition on the alternate shutdown system and their

r

contribution towards poor material conditions and housekeeping in the "B" emergency core

l

cooling system (ECCS) corner room. These actions were the result of a failure to follow

l

procedures and were assessed as a violation of Technical Specification requirements, as specified

,

'

in the enclosed Notice of Violation.

l

We have also highlighted a finding regarding your application of the NRC policy and work hour

guidelines described in Generic 12tter 82-12, " Nuclear Power Plant Staff Working Hours." We

request that you respond, in writing, with your intended administrative controls for overtime in

-

outages.

In accordance with 10 CFR .2.790 of the NRC's " Rules of Practices," a copy of this letter and

the enclosed Notice will be placed in the NRC Public Document Room. You are required to

respond to this letter and should follow the instructions specified in the enclosed Notice. The

responses directed by this letter and the enclosed Notice are not subject to the _ clearance

procedures of the Office of Management and Budget as required by the Paperwork Reduction

Act of 1980, Public Law No. 96.511.
