Dear Mr. Knop:

INSPECTION REPORT NO. 50-483/82-19 This reply is in response to your letter of January 20, 1983 which transmitted the report of the inspection conducted at Callaway Plant, Unit 1 during the period of November 1 to December 31, 1982.

Our response to the items of noncompliance are presented below in the order listed within the body of inspection report number 50-483/

82-19.

None of the material in the inspection report or in this response is considered proprietary by Union Electric Company.

(50-483/82-19-01) SEVERITY LEVEL V VIOLATION 10 CFR 50, Appendix B, Criterion X,

states, "A program for inspection of activities affecting quality shall be established and executed...

to verify conformance with the documented instructions, procedures and drawings for accomplishing the activity."

Daniel International Corporation Procedure QCP 100, Revision 20, Section 4.1, requires the Civil Quality Inspector, in Step 4, to:

" Inspect surface mounted plate for correct hole diameter."

Contrary to the above, a surface mounted plate, Log 15746, was installed, inspected, and accepted although one hole in the plate had been reamed to a diameter exceeding the allowable diameter specified by the designer.

Corrective Action Taken And The Results Achieved:

The bolt hole in question for PEIF 15746 was nonsymmetrical.

If the oblongation existed at the time of the initial visual inspection it should have been detected at that time, since the visual inspection includes checking the bolt hole diameter.

' Mr. R. C. Knop February 18p 1983 report'for PEIF 15746 indicates all hole diameters were acceptable.

It is therefore concluded that the reaming occurred between the time the plate was visually inspected and the actual bolt torquing

.of the installed plate.

The corrective action-taken and results are as follows:

1) NCR number 2SN-6980-C was written to document the nonconfor-mance.

The NCR was dispositioned Use-As-Is and has been closed.

2) A review of pertinent records indicates that all six (6) of the plates mentioned'in the allegations were installed by the second shift Ironworkers between the dates of 7-8-82 and 8-23-82.

A count of plates completed between these dates showed 73 plates had been installed during this period by the second shift Ironworkers.

A reinspection of 20 (27%)

.was done by DIC Quality Inspectors and all plates passed reinspection.

Of these twenty PEIF's nuts and washers were removed from ten to verify acceptable bolt hole diameters.

The plates reinspected include the following:

  • 15732
  • 16200 15864
  • 16230
  • 15842
  • 16239 16022 16234 15945 15689
  • 16053
  • 16236
  • 16182
  • 15726 16114
  • 16252 16062 15986 16201 16336
  • Indicates PEIF's which had nuts and washers removed for hole diameter inspection.

These PEIF's were selected at random from the 73 PEIF's installed between 7-8-82 and 8-23-82.

Based on the above information, the reaming of plate 15746 is considered to be an isolated case.

Corrective Action To Be Taken To Avoid Further Noncompliance:

Training sessions have been held by Civil Engineering for the craft workers concerning the installation of plates and quality work.

The training session also covered the requirement that any modifications to QC inspected work must be reinspected prior to further work which might preclude reinspection of the modification.

Date When Full Compliance Will Be Achieved:

Training sessions have been completed.

Full' compliance.was achieved on February 3, 1983.

C. Knop February 18, 1983 (50-483/82-19-02) SEVERITY LEVEL V VIOLATION 10 CFR 50, Appendix B, Criterion V, states, " Activities affecting quality shall be prescribed by documented instructions, procedures or drawings...and shall be accomplished in accordance with these instructions, procedures and drawings."

Bechtel Drawing E-OR8900, Section 3.36. (a), states, " Minimum separation between conduit systems or between tray / conduit shall be one inch.

This minimum separation is typical for all areas."

Contrary to the above, minimum separation distance is not maintained between redundant Class IE flexible conduits at feedwater flow control valves.

Corrective Action Taken And The Results Achieved:

Investigation of conduits 1U4025-4U4031 and 4U4029-lU4024, located in the Turbine Building, revealed that they had been installed correctly but that the flexible conduit could be easily moved to violate the separation criteria.

Spacers have now been added to the conduit to prevent this possibility.

Other safety-related conduit in the Turbine Building was also inspected; no other sepa-ration violations were noted.

This action was completed on January 6, 1983.

The Construction Manager issued a project memorandum on February 5, 1983 to alert craft supervision of their responsibility to observe conduit separation.

Corrective Action To Be Taken To Avoid Further Noncompliance:

Surveillances will be performed on those areas of the plant where different separation groups of flexible conduit have the potential of violating separation requirements.

Date When Full Compliance Will Be Achieved:

Full compliance was achieved on January 6, 1983.
