On January 5,1998, at approximately 2320 hours0.0269 days <br />0.644 hours <br />0.00384 weeks <br />8.8276e-4 months <br />, guard force petsonnel making routine security checks of classified containers discovered an unattended and unlocked security container housit ;

classified matter. The container was located in the Nuclear Materials Accounting section of the X-100 Administration Building. - An accountant, one of the three authorized custodians of the container, was immediately notified at his residence.

On January 6,1998, at approximately 0015 hours1.736111e-4 days <br />0.00417 hours <br />2.480159e-5 weeks <br />5.7075e-6 months <br />, the custodian arrived on plantsite. The officer ana the custodian properly secured the container. At approximately 0630 hours0.00729 days <br />0.175 hours <br />0.00104 weeks <br />2.39715e-4 months <br />, a second custodian, an accounting specialist, inventoried the contents of the container and confirmed that none of the classified matter was missing.

-On January 6,1998, a security investigator conducted interviews with the Nuclear Materials Accounting Section Manager, who is also a custodian of the container, and the accounting specialist L

plantsite. She stated before departing the area, the sect'on manager told her to leave the container open and that he would secure the container at tiie conclusion of the work day. A review of the SF-702, " Security Container Check Sheet" on Jarcary 5,1998, confinns the accounting specialist had opened the container at 0630 hours0.00729 days <br />0.175 hours <br />0.00104 weeks <br />2.39715e-4 months <br /> and that the container was not closed or checked.

United States Nuclear Regulatory Commission Page Two January 22,1998 The section manager concurred with the accounting spe :Jist and stated that while preparing to leave the building his concentration was diverted and he neglected to secure the conteiner. The manager stated that he accepted full responsibility for not securing the container.

A review of Procedure XP2-SS-SS1039 adequately states the action steps that are to be taken to secure a classified container. The }nocedure says that when securing a classified repository, close all drawers, spin the combir.ation dial at least four times in either a clockwise or counterclockwise manner, test each drawer to assure that the locking mechanism is operable and initial and fill in the time that the repository is secured in the " closed by" portion of the SF-702. Following the securing of the repository, the p.ocedure requires the custodian to ask a fellow employee, where practical, to check the repository by spinning the combination dial at least four times in either a clockwise or counterclockwise manner and then test each drawer to assure the locking mechanism is operable.

These procedural steps were not followed by the scetion manager who was responsible for securing the container.

At PORTS, an unlocked security container found unattended constitutes a security infraction.

Security containers are used to protect classified information. The unlocked and unattended security i

container was within the plantsite controlled access area. Personnel having access to the controlled area mi st have the appropriate access authorization level for entry or be under positive escort of a cleared individual. Due to these facts it is believed that no unauthorized access was gained to the container and no classified information or material was compromised.

On January 5,1998, the Section Manager was issued a security infraction. The Section Manager also reviewed XP2-SS SS1039, which specifies the custodial responsibilities for the protection of classified information. The Section Manager was counseled on the correct process for securing a security container and completion of the SF-702 "Sacurity Container Check Sheet."
