DPR-42 50 306 DPR 60 Response to Inspection Reports No. 282/86011 and 306/86013 In response to your letter of January 8, 1987, which transmitted Inspection Reports No. 282/86011 and 306/86013, the following information is offered.
Violation Facility Operating License DPR 42 authorizes operation of the Unit I reactor at a core power level not in excess of 1650 megawatts thermal (1004).
Contrary to this requirement, on November 16, 1986, the Unit I reactor was operated at a calculated power of 101.01% and over an eight-hour shift was operated at an average power of 100.154.
In addition, Technical Specification 6.5.A.4 requires that detailed written procedures be prepared and followed for surveillance and testing requirements that could have an effect on nucicar safety.
Contrary to this requirement, existing plant procedures do not appear to be adequate to prevent overpower conditions of the type which occurred on November 16, 1986.
Response
Present procedures provide for a daily heat balance in accordance with Technical Specification requirements.
However, a daily heat balance will not preclude operation slightly above 100n for an 8 hour9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> shift.
Work tu underway which will cause the heat balance calculation to be performed by the plant process computer and displayed continuously; until this modification is in place, the units havo been administratively dorated to 99.5% power to prevent recurrence.
Full compliance has been achieved.
This event was reported as Unit 1 LER 86 006.

o era 987 Page 2 NRC Concern During excavation work being deae for a new warehouse facility outside the Protected Area, underground cables were severed. Although not a violation, it was indicative of a recurring problem with control of work activities.
Response
Additional controls over construction work onsito had boon implemented in responso to earlier ovents.
Further controls have been instituted as a result of this event. A procedure for excavation was developed (SWI 8 PI 28) and will be used in futuro excavation work. This procedure incorporates those olements that will assure work control and provent similar events.
Training on the use of this procedure will be completed by }bbruary 13, 1987.
The decision has been made to bring our Nuclear Engineering and Construction Department under the Vice President Nuclear Generation.
It la expected this action will further improve the work control process on thoso projects assigned to that department since the reporting chain will be simplified and linos of communication will bo improved.
