Subject: NOTICE OF VIOLATION

(Combined Inspection 50-245/86-26; 50-336/86-32; 50-423/86-38)

This refers to a special NRC inspection conducted on December 11-12, 1986 at

the Millstone Nuclear Power Station, Waterford, Connecticut to review the

circumstances associated with a violation of the physical security plan at

Millstone. The violation, which was identified by a member of your maintenance

staff and reported to the NRC, involved a degradation of the protected area

(PA) barrier. A copy of the inspection report is enclosed.

Since the degradation could not be easily exploited and was not readily

apparent, and the area in the vicinity of the degradation was well lit and

monitored with assessment aids, the violation, which is described in the

enclosed Notice, has been classified at Severity Level IV in accordance with

the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy) (1986). Nonetheless, the violation

is of concern to the NRC since the degradation was caused when certain equip-

ment, which comprised a portion of the PA barrier, was removed by maintenance

personnel without the security department being notified. As a result, com-

pensatory measures were not taken. This violation demonstrates the importance

of appropriate coordination with the security department when such activities

occur. Any similar deficiencies in the future may result in additional

enforcement action.

You are required to respond to this letter and the enclosed Notice, and in

preparing your response, you should follow the instructions specified in the

enclosed Notice.

In your response, you should document the specific actions

taken and any additional actions you plan to prevent recurrence.

Further, you

should place all Safeguards Information (as defined in 10 CFR 73.21) in

enclosures, so as to allow your letter without the enclosures to be placed in

the Public Document Room. After reviewing your response to this Notice,

including your proposed corrective actions, the NRC will determine whether

further NRC enforcement action is necessary to ensure compliance with NRC

regulatory requirements.

security program that have been determined to be exempt from public disclosure

in accordance with 10 CFR 73.21 (Safeguards Information). Therefore, those

portions will not be placed in the NRC Public Document Room and will receive

The response directed by this letter and the enclosed Notice is not subject to

the clearance procedures of the Office of Management and Budget as required by

the Paperwork Reduction Act of 1980, PL 96-511.
