The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the Nuclear Regulatory Commission (NRC) rules and regulations and the conditions of your license. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspector. The inspection findings are as follows:

Temporary Job Site - Barnes-Jewish Hospital (Washington University), St. Louis, MO 17. Scope and Observations:

This was an inspection of a service provider licensed by the State of Louisiana and working under reciprocity to perform a source exchange on an Elekta Instrument AB Model Leksell Gamma Knife Icon gamma stereotactic radiosurgery unit at Washington University in St. Louis, Missouri. This inspection was performed concurrently with an inspection of Washington University in St. Louis, NRC License No. 24-00167-11. Two licensee employees performed the source exchange and associated activities as well as coordinated with the shipper/receiver of the sources and other administrative tasks associated with the source exchange.

During the inspection, the inspector observed the licensee verify proper installation of the portable hot cell used for the source exchange as well as surveys of the device and the hot cell, and the use and functionality of the manipulator tools inside the hot cell. The inspector also observed the removal of multiple sources from the unit, the removal of the sources from their bushings, leak tests of the device and bushings, the installation of multiple new sources (pre-loaded in bushings), and the verification that sources were loaded into the correct locations. The inspector observed the use of personnel dosimetry and survey meters and interviewed the licensee employees regarding radiation safety practices; engineered safety features of the device, the hot cell, and the manipulator tools; and emergency procedures in case of radiological incidents. The inspector found the employees to be appropriately knowledgeable of regulatory requirements, the licensee's procedures, and radiation safety practices. The inspector also reviewed records including a sample of leak tests, source load patterns, and the licensee's source reload and maintenance procedures. The inspector also conducted independent and confirmatory surveys, and found dose rates to be comparable to the licensee's surveys.

No violations of NRC requirements were identified as a result of this inspection.
