SUBJECT: REPLYTONOTICEOFVIOL4TION-NRCINSPECTIONREPORT50-410/93-14EnclosedisNiagaraMohawkPowerCorporation'sreplytotheNoticeofViolationcontainedinthesubjectInspectionReportdatedAugust3,1993.Thediscussion,asrequestedbytheInspectionReport,ofNiagaraMohawk'spositionwithrespecttocompliancewithANSI/ANS3.2-1982,Section~~~~~~~~5.3.9.1isalsoenclosed.Thisletteralsoprovidesadditionalinformation,asrequested,regardingthequalityoftheNineMilePointUnit2OperatingProcedures.Viola'eoseWebelievethattheresponsetotheNoticeofViolation(Enclosure1)appropriatelyaddressesthecausesofthisviolationandpresentsappropriatecorrectiveactionswhichwillpreventrecurrence.-Nor/naPrcrFEnclosure2providesNiagaraMohawk'spositionwithrespecttotheissueofoff-normalprocedureformat.TheInspectionReportindicatedthatourresolutionoftheissueregardingprocedureformathasnotbeentimely.Weacknowledgethat,untilthisinspection,wemisinterpretedthefocusofthisissue,believingtheconcerntobethequalityofselectedOperatingProcedures,whichwewereaddressingbyupgradingtheElectricalOperatingProceduresandenhancingourprocedurepreparationprocesses.WediscussedourpositionregardingthisissuewiththeNRCRegionStaffduringtheon-siteinspectionscoveredbythisreport.Furthermore,asdiscussedinEnclosure2,NiagaraMohawkhasinitiatedaplanforasignificantrestructuringofUnit2OperatingProcedures(OPs)bydevelopingproceduresinanewcategory,SpecialOperatingProcedures(SOPs).TheUnit2SOPswillincorporate,asappropriate,theattributesofsymptoms,automaticactions,immediateandsubsequentoperatoractions.InanAugust6,1993lettertotheRegionalAdministrator,Mr.T.Martin,wedescribedthisplanandprovidedourimplementationschedule.Asdiscussedinourletter,implementationofthisplanwillbecompletebyJuly31,1994inaccordancewithastructuredschedulewhichwillallowadequatetimeforverificationandtraining.9309080163930903PDRADOCK050004io8PDR

E~NCLCENIAGARAMOHAWKPOWERCORPORATIONNINEMILEPOINTUNIT2DOCKETNO.50-410NPF49"REPLYTONOTICEOFVIOLATION,"ASCONTAINEDININSPECTIONREPORT50410/93-14VIOL4TION-4103-14-0110CFR50AppendixB,CriterionVI,ondocumentcontrolrequiresinpartthatmeasuresshallassurethatdocuments,suchasprocedures,includingchanges,arereviewedforadequacy.Contrarytotheabove,asofJuly8,1993,anoperatingprocedurechangewasnotreviewedforadequacyinthatOP-71DwouldhavepermittedbothlogictrainsoftheAutomaticDepressurizationSystemtobemadeinoperableduringrestorationofonepowersupplytrain.ThisisaSeverityLevelIVviolation(Supplement1).1.THEREAIRTHEVIL4TINAlimitedrevisiontoprocedureN2-OP-72(OP-72)waswritteninAugust1992toaddressalossofpowertoloadssuppliedfrom2VBA-UPS2B(UPS2B).ThisprocedurewaspreparedtosupportacorrectivemaintenanceactivityperformedinSeptember1992.ThestepsforrestoringpowertotheUPS2BloadswereextractedfromprocedureN2-PM-13,whichhadbeenpreviouslydemonstratedtobeeffectiveforde-energizingandrestoringDivisionIIpowersupplies.WhiledevelopingthelimitedrevisiontoOP-72,theprovisionsfortheuseoftheAutomaticDepressurizationSystem(ADS)inhibitswitcheswaserroneouslyaddedasanalternativetoopeningbreaker2BYS*PNL201B-10.ThiserrorwasnotidentifiedbytheQualifiedTechnicalReviewer.Subsequently,procedureN2-OP-71D(OP-71D)wasdevelopedaspartoftheElectricalOPupgradeprogram.ThesectiononrestoringpowertoUPS2BloadswastransferredfromOP-72toOP-71DwhentheupgradedElectricalOPswereimplemented.OP-71Dreceivedacross-disciplinaryandtechnicalreviewpriortotheadditionofthesectiononrestoringpowertoUPS2Bloads.ThereviewbytheQualifiedTechnicalReviewer,afterincorporatingthissectionfromOP-72,didnotidentifytheerror.Theprimarycausefortheincorrectprocedurehasbeendeterminedtobeinadequatemanagerialmethodsinthatprocedureprogramrequirementsandmanagementexpectationsregardingprocedurepreparationandreviewactivitieswerenotwellunderstoodbytheindividualsinvolved.Inaddition,managementoversightwaslessthanadequateinidentifyingthattheseexpectationswerenotwellunderstood.Page1of3 0
TheproceduresectionaddressingtherestorationofUPS2Bloadshasneverbeenused.Furthermore,hadthisprocedurebeenused,theinternalinconsistencyresultingfromtheerroneousstepwouldinalllikelihoodhaveresultedinanoperatordecisiontostoptheprocedureandtoinitiateactiontoresolvethediscrepancy.Additionally,operationoftheADSinhibitswitcheswouldhaveinitiatedControlRoomfrontpanelannunciators,alertingtheoperatingshiftoftheneedtotakeactiontorestoreADSoperability.2.RRETIVEACTINTAKENANDTHERELTAHIEVEDAnimmediateProcedureChangeEvaluation(PCE)wasissuedinresponsetotheInspectoridentifiedproceduredeficiency.ThisPCEremovedtheoptiontousetheADSswitchesduringtheperformanceofthisprocedure.DeviationEventReport(DER)¹2-93-1611wasissuedonJuly8,1993,todocumenttheevaluationofthecauseofthisproceduredeficiencyandcorrectiveactions.AsampleofpreviouslyrevisedOperationsDepartmentproceduresisbeingreviewedfortechnicalaccuracy.TheresultsofthatreviewwillbepresentedtotheSiteOperationsReviewCommittee(SORC)forconsiderationindeterminingwhetheradditionalcorrectiveactionsarewarranted.AsampleofoneprocedureperQualifiedTechnicalReviewerwillbereviewedintheinitialsample.AtthistimeeightupgradedOperationsDepartmentprocedureshavebeenreviewedwithnoerrorsintechnicalcontentidentified.Thesereviewsdididentifysomeeditorialerrors.3.RRETIVEACTIONTBETAKENTOAVIDFURTHERVIOLATINNiagaraMohawkwillcontinueupgradingourUnit2OPsonaprioritizedbasis.SeniorOperationsDepartmentSupervision(GeneralSupervisorlevelorhigher)willreviewselectedrevisedprocedurespriortopublicationwiththeprocedureauthorandQualifiedTechnicalReviewertoensuretheprocedurepreparationandreviewactivitieswereperformedinfullcompliancewithprogramrequirementsandmanagementexpectations.OperationsLineManagementwillreinforce,withprocedureauthorsandQualifiedTechnicalReviewers,theexpectationsandrequirementsforproperprocedurepreparationandTechnicalReviewactivities.TheOperationsBranchManagerwillmeetwithallOperationsDepartmentpersonneltoreinforceNuclearDivisionStandardsofPerformanceandexpectationsregardingtheemphasisonsafetyandqualityofworkhavingpriorityabovescheduleadherenceaswellasreinforcingmanagementexpectationsonprocedurepreparationandreviewactivities.TrainingtoemphasizetherolesandresponsibilitiesofTechnicalReviewerswillbeincludedintheTechnicalStaffandReactorOperatorcontinuedtrainingprograms.AugmentedQualityAssurancesurveillanceactivitieswillbeutilizedtomonitortheeffectivenessoffutureOperationsDepartmentprocedurerevisionactivities.ThesesurveillanceactivityresultswillbefedbacktoOperationsDepartmentManagementtogaugetheeffectivenessofthecorrectiveactionstaken.Page2of3

ENCLOSURE2NIAGARAMOHAWKPOWERCORPORATIONNINEMILEPOINTUNIT2DOCKETNO.50-410NPF49NIAARAMOHAWKPOSITIONONFRMATOFFF-NORMALPRCEDUR~fNDUCNBaseduponthecommentsinInspectionReport93-14,itappearsthatNiagaraMohawkandtheNRCStaffhavedifferentpositionsregardingtherequiredformatfor"off-normal"procedures.ThisEnclosurediscussesNiagaraMohawk'spositiononthisissue.~C*NANiagaraMohawkbelievesthatthecurrentUnit2procedureprogramisincompliancewithANS3.2-1982.InSection13.5oftheUSAR,NiagaraMohawkcommitstofollowSection5.3ofANS3.2-1982.Insubsequentsubsections,however,wenotethatourEOPswillfollowRev.4oftheBWROGformatforsymptombasedEOPs.Thispositionisconsistentwithfootnote13toSection5.3.9.1ofANS3.2.ANS3.2Section5.3.9.2,whichrequiresprocedurestorespondtopotentialemergencies,doesnotspecifyaparticularformat.AtthetimetheUnit2OPswereinitiallyprepared,theyfollowedthegeneralformat,styleandcontentthenbeingusedbytheexistingUnit1procedures.Furthermore,NiagaraMohawkspecificallydecidedtoincorporateoff-normalactionswithintheOPstominimizethenumberofproceduresthatoperatorswouldneedtoreference.ThepotentialemergenciesdiscussedinSection5.3.9.2andAppendixA10ofANS3.2arecoveredbytheoff-normalsectionofourOPsorbyotherapprovedsiteprocedures.Subsequentexperience,however,hasdemonstratedthedesirabilityofhavingstandaloneproceduresforrespondingtoselectedoperatingevents.Asdiscussedbelow,wehavedevelopedacomprehensiveplantodevelopthesestandaloneproceduresandprovidedthisinformationtotheNRCinourAugust6,1993,lettertotheRegionIAdministrator.FF-NRMALPREDREPLANADTATNiagaraMohawk'splanistodevelopanewsetofUnit2procedurescalledSpecialOperatingProcedures,SOPs,whichwillcovereventsofpotentialemergency.AstheseSOPsareprepared,theappropriateoff-normalsectionsofourcurrentOPswillbemodified.Wearedevelopingawriter'sguideforSOPsandtheOperationsStaffisdevelopingtheformatbestsuitedforSOPs.Insomecases,thismayresultintheuseofflow-charttypeprocedures,Page1of2
similartotheEOPs.Regardlessoftheformat,theattributesofsymptoms,automaticactions,immediateandsubsequentoperatoractions,willbeincorporatedintheindividualprocedures.Incorporatingtheseattributesmaynotnecessarilyinvolvespecificproceduresectionswiththeaboveheadings.Forexample,automaticactionsmaybeincorporatedasaverificationstepattheappropriatetimewithintheimmediateorsubsequentoperatoractionsections.TheUSARwillberevisedtoreflecttherevisedprocedureprogram.$UMMARYNiagaraMohawkbelievesthattheexistingUnit2procedureprogramisincompliancewiththelicensingbasis,however,NiagaraMohawkwillrevisetheUSARtomoreclearlyandspecificallydescribeourprocedureprogramandcommitmenttoANS3.2.WehavealsoconcludedthatasetofSpecialOperatingProcedureswouldbeanenhancementtoourprogramandhavedevelopedaplantoimplementthatapproach.Page2of2
