'Re:St.LucieUnits1and2DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort94-11eVIOLATION:10CFR50AppendixB,CriterionXVI,requiresmeasureswhichassurethatconditionsadversetoqualityare,promptlyidentifiedandcorrected.Forsignificantconditions,theidentification,cause,andcorrectiveactiontaken'rerequiredtobedocumentedandreportedtoappropriatelevelsofmanagement.Contrarytotheabove,onMay3,1994,inthreeinstancesthelicensee'smeasureshadnotbeeneffectiveinassuringthat:(1)conditionsadversetoqualitywouldbepromptlyidentifiedandcorrected;and(2)theidentificationandcorrectionofsignificantconditionsadversetoqualityweredocumentedandreportedtomanagement.ThesignificantconditionsadversetoqualityinvolvedstallfailuresofredundantUnit2motor-operatedvalves(MOVs)2-MV-08-12and-13,whichoperatetosupplysteamtotheUnit2TurbineDrivenAuxiliaryFeedwaterPump.Valve2-MV-08-12'stalledwhenactuatedtoopenonNovember11,1991,and2-MV-08-13similarlystalledonSeptember15,1992andJuly20,1993.Thelicenseedidnotidentifythatthestallthrustandtorqueforces.hadexceededtheassociatedactuatorandvalvemaximumlimits.Correctiveactionsdocumenteddidnotprovideanadequatebasisforreturningtheactuatorandvalvetooperation.RESPONSE:1~REASONFORV10LATIONPlantMaintenanceProcedure0950065"TroubleshootingLimitorqueValveActuators"didnotincludetherecommendationsof"LimitorqueMaintenanceUpdate92-1"and"LimitorqueTechnicalUpdate92-1."Thesedocumentscontaincalculationsusedtodeterminetheapproximateloadstheactuatorsandvalvesmayhaveseenunderstallconditions.Performingthesecalculationshelpindeterminingifdamagehasoccurredbasedonallowableoverloads.Thiswaserroneouslydeemedunnecessarybyelectricalmaintenanc'eandengineeringpersonnelduetothecapabilityoftheMOVdiagnosticsequipmenttopredictproblemswiththecomponentsofconcernduringpotentialstallevents.Therecommendedcalculationswereperformedatthetimeofthisinspectionandprovedthatthestallthrustandtorqueforceshadnotexceededtheassociatedactuatorandvalvemaximumlimits.2~CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDThecompleteactuatorassemblyandvalveinternalsofMV-08-12andMV-08-13werereplacedonApril15,1994toensureincreasedreliability.Nofurtherproblemshavebeenexperiencedwiththesevalve '0Re:3~St.LucieUnits1and2DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort94-11CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSPlantMaintenanceProcedure0950065"TroubleshootingLimitorqueValveActuators"hasbeenchangedtoincludeareferencetotherecommendationsof"LimitorqueMaintenanceUpdate92-1"and"LimitorqueTechnicalUpdate92-1."ThiswillensurethatthenecessarycalculationsareperformedanddocumentedwhenaMOVhasstalled.4~DATEWHENFULLCOMPLIANCEWILLBEACHIEVEDFullcompliancewasachievedonApril15,1994withthecompletionofcorrectiveaction2above.
