1.THEREA(Continue@RTHEVILATIAtapproximately0130hours,theRefuelFloorRadiationProtection(RP)Technicianwasinformedbytheforemanofthecontractorgroupworkingonthereactorvesselstudprotectorsthatwirebrushinghadbegunonthereactorvesselstuds.TheRPTechnicianhadnotexpectedthatwirebrushingwouldbeperformedsincetheRPChiefTechnicianhadlimitedworktoremovalofthestudprotectorsinordertopermittheRPTechniciantoperformasurveyonthestudspriortowirebrushing.TheRPTechnicianknewthattheprotectiveclothingforthecavityworkprescribedbytheRWPwasappropriatetoprovideadequateprotectionfortheworkersinthecavity.Thus,afterbeingnotifiedthatwirebrushinghadbegun,hisimmediateconcernwasairsamplingtoverifytheairborneconcentrationforpersonnelontherefuelfloorwhowerenotwearingrespirators.AstheRPtechnicianwassettinguptheairsampleratthecavityedge,therefuelfloorcontinuousairmonitoralarmedduetothewirebrushingactivity,andtheRPtechnicianorderedanevacuationofallpersonnelfromtherefuelfloor.AstheRPtechnicianassistedtheworkersexitingthecavityinremovingtheiroutersetofprotectiveclothing,hewasadvisedthatoneworkerwaswearingonlyonesetofcoveralls.Thisindividualwasfoundtobecontaminatedabouttheface,neck,chest,andrightelbow.Analysisoftheairsampleresultsshowedapeakairborneradioactivityconcentrationof63%oftheMaximumPermissibleConcentration(MPC)valuesof10CFR20.Laterthatevening(thenextworkdayfortheshiftinvolved),anaccountabilitymeetingwasheldtodiscusstheevent.InattendancewereRPsupervision,contractorsupervision,andtheworkerwhowasdoingthewirebrushingonthereactorvesselstudswithouttherequiredprotectiveclothing.ThecontractorsupervisorindicatedthathehadgivendirectiontotheworkertobeginwirebrushingthestudswithouthavingfirstcontactedRP,asrequiredbytheRWPandthepre-jobbriefingwiththeRPChiefTechnician.TheworkeradmittedtowearinginappropriateprotectiveclothingandtonothavingreadtheRWP.RadiationProtectionroutinelyperformsreviewsofradiologicalworkpracticesusingprocedureS-RAP-RPP-0106,"ReviewofRadiologicalWorkPractices".SinceOctober1,1991,veryfewinstancesofRWPnon-compliancehavebeennoted.QuarterlyRadiationProtectionperformsprocedureS-RAP-RPP-0108,"RadiationProtectionSelfAssessment".TheyhavenotidentifiedanytrendsofRWPnon-compliance.ThesereviewsincludecontractorsaswellasNiagaraMohawkemployees.Therefore,thisinstanceofRWPnon-complianceisdeemednottobeprogrammatic.Thus,thecauseofthiseventwasdeterminedtobepersonnelerror.ThesupervisorfailedtofollowthedirectionoftheRPChiefTechnicianbyinstructingtheworkertogoaheadandperformthewirebrushingandtheworkerfailedtoread,understandandcomplywithRWPrequirements.002623LL(2)

2.-RRETIVEATITAKEATHERTAHIEVEDImmediatecorrectiveactionsinresponsetothiseventwere:~Restrictingaccesstotherefuelflooruntilradiologicalsurveyswereperformed,postingsandentryrequirementsupdated,andareasdecontaminated.~Decontaminatingtheindividualwhowasfoundtobecontaminatedabouthisface,neck,chest,andrightelbow.AContaminationOccurrenceReport(COR)waswrittenforthisperson.Thecontaminationlevelwasbelowthecriteriaforperformingaskindosecalculation.~Providingawholebodycountforthecontaminatedworkerandhisco-worker.Basedontheresults,theworkerwasassignedaninternalintakeofapproximatelytwoMaximumPermissibleConcentration-hours(MPC-hrs.)duetoZinc-65.Theco-workerhaddetectablebutaninsignificantradioactivitylevelduetoZinc-'5.3.~Holdinganaccountabilitymeetingtodeterminethecauseofnotfollowingthe.procedure.Theresponsibilitiesofworkers,foremenandsupervisionwithrespecttopropernotificationtoRPofspecificworktobeperformedandobservanceofRWPandRPdirectiveswerereviewedwithallpresent.RRETIVEATITBETAKENTAVIDTHERVILATINCorrectiveactionstobetakenare:AreaCoordinatorsassignedtokeyradiologicalareasduringthenextrefueloutagewillberesponsibleforachievingestablishedradiologicalperformancegoalsfortheirrespectiveareas.Thesegoalswillincludeworker'sradiologicalperformalice.SelectedcontractsinvolvingextensiveworkinradiologicalareaswillreceivereviewandapprovalbyRadiationProtectionBranchManagersbeforeissuance,toestablishtheneedforpenaltyclausesand/orothermeasuresasappropriatewithrespecttocontractworkerradiologicalperformance.~Accesscontroltokeyradiologicalareaswillbeincreasedduringthenextrefueloutage.TheseareaswillbeincludedintheRefuelOutage¹3Pre-OutageALARAchecklistbasedonradiologicalrisk.AnaccesscontrolpersonforthesekeyareaswillassistworkersbyverifyingtheirunderstandingandcompliancewithRWPrequirements.~ThiseventwillbediscussedinGeneralEmployeeTraining,withemphasisplacedonthesignificanceofthisevent-andradiationworkeraccountability.002623LL(3)
