Peach Bottom Atomic Power Station Units 2 and 3 Response to Combined Inspection Reports No. 50-277/87-22; 50-278/87-22 and 50-277/87-25; 50-278/87-25 Concerning Shutdown Cooling Isolations
Dear Mr. Kane:
Your letter dated October 2, 1987 transmitted Combined Inspection Report Nos. 50-277/87-22 and 50-278/87-22 for the inspection period from July 18, 1987 to August 28, 1987.
I Additiorally, your letter dated November 5,1987 transmitted Combined Inspection Report Nos. 50-277/87-25 and 50-278/87-25 for the inspection p3riod from August 29, 1987 to October 16, 1987. These inspection reports identified a concern with respect to the number of shutdown coolitig isolations that occurred during the above identified inspection per!.ods and requested that we analyze the root causes and provide corrective actions to reduce the frequency of occurrences.
In our previous correspondence related to these inspection reports (dated January 11, 1988) we stated that a formal root cause evaluation was being undertaken and would be submitted.
Attached is our report entitled "Root Cause Investigation of Shutdown Cooling Isolations/ Peach Bottom Atomic Power Station", which is being submitted in accordance with our commitment. Identified in this report are causal factors associated with the shutdown cooling 1 isolations. Philadelphia Electric is currently evaluating these causal factors and identifying appropriate actions to be taken in response to each. The results of this evaluation and the corrective actions planned or taken will be provided to the NRC by June 10, 1988.
