| Title | Inadequate Assessment of Pressure Locking and Thermal Binding of the RHR Suction Isolation Valves from the Suppression Pool |
| Description | The inspectors identified a finding of very low safety significance and associated NCV of 10 CFR Part 50, Appendix B, Criterion III, Design Control, for the licensees failure to adequately assess the susceptibility to pressure locking and thermal binding of the RHR suction isolation valves from the suppression pool. Specifically, the design reviews for susceptibility to pressure locking and thermal binding did not consider the operational configuration of these valves when the RHR system is operated in the shutdown cooling mode. This finding was entered into the licensees CAP to reconcile the licensing requirements and design of the RHR system. The performance deficiency was determined to be more than minor because it was associated with the Mitigating System Cornerstone attribute of equipment performance and adversely affected the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. In addition, the finding was associated with the Containment Barrier Cornerstone attribute of structures, systems, components, and barrier performance and adversely affected the cornerstone objective of providing reasonable assurance that physical design barriers protect the public from radionuclide releases caused by accidents or events. The finding screened as of very low safety significance (Green) using a Phase II evaluation. Specifically, all the core damage sequences affected were calculated to have a frequency of 1x10-8 per year or less. The inspectors determined the cause of this finding did not represent current licensee performance and, thus, no cross-cutting aspect was assigned. |
