APPENDIX NOTICE OF DEVIATION Public Service Company of New Hampshire Docket No. 50-443 Seabrcok Unit 1 License No. CPPR-135 As a result of the inspection conducted on August 28 - October 18,1985, it appears that one of your activities was not conducted in accordance with your commitments in the Seabrook Station Final Safety Analysis Report (FSAR), as indicated below:
The Seabrook FSAR, in Section 1.8, specifies conformance to the provisions of USNRC Regulatory Guide (RG) 1.97. Although certain exceptions to RG 1.97 are discussed in the FSAR, none of them involve either the Reactor Vessel Level Indication System (RVLIS) or the Reactor Coolant System (RCS)
wide range pressure signals used for safety-related valve interlocks. By letter (SBN-864) dated August 30, 1985, Public Service Company of New Hampshire confirmed to the USNRC, Seabrook Station's commitment to RG 1.97, Revision 3, and provided detailed information to be incorporated into the FSAR by a future amendment. Included in this letter is an Accident Monitoring Instrumentation List which indicates that the RCS wide range pressure transmitters and RVLIS level transmitters are considered RG 1.97 Design Category 1 instruments. RG 1.97, Revision 3, provides that the quality assurance applied to Design Category 1 instrumentation be commen-surate with nuclear Quality Assurance (QA) (i.e., ANSI /ASME) criteri Contrary to the above, as of September 10, 1985, tubing installation to both the RCS wide range pressure transmitters and the RVLIS level trans-mitters had proceeded under QA program controls not commensurate with nuclear QA criteria. Tha QA program (QAS-5) in use was intended for nonsafety-related components and meets RG 1.97 Design Category 2 criteria, but does not meet the Design Category 1 criteria committed to for the subject RVLIS and RCS wide range pressure instrument This is a deviation, applicable to Docket No. 50-44 Public Service Company of New Hampshire is hereby requested to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each deviation: (1)
the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further deviations; and (3) the date when corrective action will be completed.
