ENCLOSURE 1 NOTICE OF VIOLATION Carolina Power and Light Company Docket No. 50-325 Brunswick 1 License No. DPR-71 During the Nuclear Regulatory Commission (NRC) inspection conducted on October 5-31, 1986, a violation of NRC requirements was identified. The violation involved failure to correctly perform a temporary procedure change. In accordance with the " General Statement of Policy and Procedure for NRC Enforce-ment Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:
Technical Specification 6.8.2 requires that temporary changes to procedures of specification 6.8.1 may be made provided that the change is approved by two members of the plant staff, at least one of whom holds a Senior Reactor ;
Operators license on the unit affected and that the change is documented, reviewed pursuant to specification 6.5.2.1 and 6.5.2.2, and approved by the General Manager - Brunswick Plant or his designated alternate within 14 days of implementation.
Technical Specification 6.8.1.a requires written procedures for the applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, November 1972. Appendix "A" item D.28 recommends procedures for the Reactor Protection System. The Unit 1 Reactor Protection System Operating Procedure, OP-03, Revision 4 implements this requirement.
Contrary to the above, an improper temporary change was made to OP-3 in that, on September 19, 1986, a step was added to section 7.0 to'open the EPA breakers. This change was made without the proper review, documentation, or approval and resulted in an inadvertent deenergizing of Reactor Protection System Bus 2A.
This is a Severity Level V violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Carolina Power and Light Company is hereby required to submit to this office within thirty days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including: (1) admission or denial of the violation (2) the reasons for the violation if admitted, (3) the corrective steps which have been taken and the (4) the corrective steps which will be taken to avoid further resultsachieved(5)thedatewhenfullcompliancewillbeachieved.
