ENCLOSURE 1

NOTICE OF VIOLATION

Florida Power and Light Company Docket No. 50-389

St. Lucie Unit 2 License ."o. NPF-16

The following violation was identified during an inspection conducted on

June 11 - July 8,1985. The Severity Level was assigned in accordance with the

NRC Enforcement Policy (10 CFR Part 2, Appendix C).

10 CFR 50, Appendix B, Criterion V, as ' implemented by section 5.0 of FP&L

Topical Quality Assurance Report, requires, in part, that activities

affecting quality be prescribed by documented instructions, procedures

and drawings and shall be accomplished in accordance with these instructions,

' procedures and drawings.

Contrary to the above, plant modifications were performed on Unit 2 in 1984

per plant change / modification (PC/M)-015-283 which added redundant over-

current devices to electrical distribution systems to comply with NRC

Regulatory Guides 1.63 and 1.75. The addition of these devices resulted in

some safety-related relays no longer being testable with the unit at power.

On June 26, 1985, an attempt was made to perform Operating Procedure (0P)

2-0400053, Engineered Safeguards Relay Test, for Semi-annual Surveillance.

The PC/M did not modify the procedures to prevent testing of certain relays.

This resulted in a plant transient that almost tripped the reactor.

This is a Severity Level V violation (Supplement I).

Pursuant to 10 CFR 2.201, you are required to submit to this office within 30

days of the date of this Notice a written statement or explanation in reply

including: (1) admission or denial of the alleged violations; (2) the reasons

, for the violations if admitted; (3) the corrective steps which have been taken

and the results achieved; (4) corrective steps which will be taken to avoid

further violations; and (5) the date when full compliance will be achieved.

Security or safeguards information should be submitted as an enclosure to

facilitate withholding it from public disclosure as required by 10 CFR 2.790(d)

or 10 CFR 73.21.
