The Limiting Condition for Operation (LCO)specified per FSAR 16.3.3.2 requires seismic monitoring instrumentation to be Operable at all times.With a required instrument inoperable for more than 30 days, Action A applies.It states,"With one or more of the above required seismic monitoring instruments inoperable for more than 30 days, prepare and submit a Special Report to the Commission within the next 10 days outlining the cause of the malfunction and the plans for restoring the instrument(s) to OPERABLE status."Cause of the Seismic Monitoring Instrument Inoperability Seismic instrument SGAR0005, Containment Structure Peak Recording Accelerometer, was removed from service on 04/12/2016 for a routine surveillance and calibration.

SGAR0005 was found to be out of tolerance for both the Transverse and Vertical measurements.

The instrument was returned to service the same day.However, subsequent review of the amount of drift in these two scratch arms, as well as the previous maintenance history of this instrument indicates a significant likelihood that this instrument was inoperable for greater than 30 days prior to its routine surveillance and calibration, thus exceeding the 30-day threshold for submittal of a Special Report.Several additional Peak Recording Accelerometers, including SGAR0002 through 4 and SGAR0006 through 8, were available during this period of likely inoperability to provide an alternate means for monitoring during a seismic event.This instrumentation ensures that sufficient capability was available to promptly determine the magnitude of a seismic event and evaluate the response of those features important to safety (thus to determine if plant shutdown is required pursuant to Appendix A of 10 CFR Part 100).Plans for Restoring the Instrument to OPERABLE status As noted above, SGAR0005 was returned to service on 04/12/2016.

Current plans are to replace this instrument during Refuel 22, which is planned for the fall of 2017.
