ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OFVIOLATIONThis is to inform the Commission that a Notice of Violation associated with a White significancedetermination process finding will be issued on or about April 7, 2006, to Tennessee ValleyAuthority as a result of inspections at the Watts Bar Nuclear Power Plant. This White finding, an issue with low to moderate increased importance to safety, involved a challenge to reactor coolant system (RCS) integrity by multiple pressurizer power-operated relief valve (PORV)actuations and a challenge to RCS inventory control by the loss of RCS coolant via the openPORVs which occurred on February 22, 2005, during transition to solid plant operations. From a defense-in-depth perspective, unlike most other accident initiators that can lead to core damage, a low temperature overpressure event could result in the reactor pressure vessel being unavailable for either subsequent recovery of the reactor core or as an additional barrier for fission product retention.A Notice of Violation is included because this White finding was associated with a violation ofTechnical Specification 5.7.1.1 and Procedure GO-6, Unit Shutdown from Hot Standby to ColdShutdown, in that TVA personnel failed to slowly raise charging flow to fill the pressure at lessthan 30 gallons per minute a required by procedure.It should be noted that the licensee has not been specifically informed of the enforcementaction.
