NRC Inspection Report Nos.
50-275 and 50-323/92-16 Gentlemen:
This refers to the routine inspection conducted by M. Miller and P. Morrill during the period April 21 through June 1,
1992.
This inspection examined your activities as authorized by NRC License Nos.
At the conclusion of the inspection, the inspectors discussed their findings with members of the PG3(E staff.
Areas examined during this inspection are described in the enclosed inspection report.
Within these areas, the inspection consisted of selective examina-tions of procedures and representative records, interviews with personnel, and observations by the inspectors.
Based on the results of this inspection, it appears that certain of your activities were not conducted in full compliance with NRC requirements, as set forth in the enclosed Notice of Violation (Notice).
This violation indicates a weakness in your control of lifting and rigging devices for heavy loads, particularly in light of a rigging problem last year involving a loss of offsite power.
It appears that better preplanning would have avoided this incorrect substitution of lifting equipment which created a potential person-nel hazard while transferring significant amounts of radioactive material.
Two non-cited violations were also identified.'
You are required to respond to this letter and should follow the instructions specified in the enclosed Notice when prepar'ing your response.
In your response, you should document the specific actions taken and any additional actions you plan to prevent recurrence.
After reviewing your response to this Notice, including your proposed corrective actions and the results of future inspections, the NRC will determine whether further NRC enforcement action is necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790(a),
a copy of this letter, the enclosures, and your response will be placed in the NRC Public Document Room.
This letter and its enclosures are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, Pub.
