ROCHESTEROASANDELECTRICCORP.DATEAPril28,1988ToMr.WilliamT.RussellSHEETNO.Contrarytotheabove,onFebruary15'988'tapproximately1830,agroupoffourindividualswasnotedworkingonthePressurizerintermediateplatform,apostedHighRadiationAreainthecontainment.Thegroupdidnothavearadiationsurveymeteroralarmingdosimeterinitspossession;alsonoqualifiedhealthphysicist,responsibleforprovidingpositivecontrolandperformingrequiredradiologicalsurveys,wasinthearea.Additionally,nohealthphysicsperiodicsurveillancefrequencywasspecifiedonthecontrol-lingworkpermit.Weagreewiththisviolationasstated.OntheafternoonofFebruary15,1988,theworkinprogressonthepressurizerintermediatelevelwastheerectionofanisolationtentfortheremovalofasbestosinsulationfromthepressurizershell.Priortothestart,ofthisjob,RG&EpipefittershadcompletedrepackingRV-203onthepressurizerintermediatelevel.Airsamplesweretakenduringthevalveworkanduponcompletionofthejob,asmearsurveywasperformed.Theresultsofthesurveyindicatedthatthecontaminationlevelsweregenerallyhigh(40-80,000dpm/100cm),withtwosmearsdirectlyunderthevalveindicating>100,000dpm/100cm.Sincetheadministrativelimitsforcontaminationapplywhenthe"generalarea"contaminationlevelsare>100,000dpm/100cm,thetechniciandidnotchangethepostingfrom"ContaminatedArea".Thiswasfurthersupportedbytheairsampleresultsfromthereliefvalveworkwhichwerewellbelowmaximumpermissibleconcentrationsforairbornecontamination.ThedocumentedsurveyresultswerepassedoutofthecontainmentareasometimeduringthelatemorningorearlyafternoonandplacedontheHealthPhysics(HP)TechnicianForeman'sdesk.Theforeman,notingthetwosmearresults>100,000dpm/100cmdecidedtohavetheareapostingupgraded.ThiswasaccomplishedbyphoningoneoftheRG&Eleadtechsassignedtothesteamgeneratorinspectionandrepairjob.Atapproximately1600hours,thecontaminatedareapostingwaschangedto>>AirborneRadioactiveMaterial"and"RespiratoryEquipmentRequired".At1615hours,theInsulatorForemanreportedtotheHPTechnicianofficetoperformapreliminaryinspectionofthepressurizerintermediatelevel.TheContractTechnicianForemanassignedatechnician(TechA)toprovidecontinuousmonitoringinlieuofanalarmingdosimetersincetheInsulatorForemanwouldonlybeintheareaalimitedtime.(Thisisallowedbyplantprocedure.)

Theresultsofthesurveyindicated
thatthecontamination
levelsweregenerally
high(40-80,000dpm/100cm),withtwosmearsdirectlyunderthevalveindicating
>100,000dpm/100cm.Sincetheadministrative
limitsforcontamination
applywhenthe"generalarea"contamination
levelsare>100,000dpm/100cm,thetechnician
didnotchangethepostingfrom"Contaminated
Area".Thiswasfurthersupported
bytheairsampleresultsfromthereliefvalveworkwhichwerewellbelowmaximumpermissible
concentrations
forairbornecontamination.
