LICENSEE-IDENTIFIED Violation

Description

Unit 1 TS 6.8.1 requires, in part, that written procedures be established, implemented, and maintained recommended in Regulatory Guide 1.33, Quality Assurance Program Requirements, which states in part that maintenance procedures for safety related equipment should provide proper instructions appropriate for the circumstances. Administrative procedure ADM-17.25, Plant Barrier Control, provides the requirements for obtaining a Breach Permit and implementing compensatory measures when an opening or removal of a controlled barrier is planned. Contrary to this, Unit 1 Door RA-8 was breached and no compensatory actions were implemented. This resulted in the Emergency Core Cooling Area Ventilation System being breached for 39 hours4.513889e-4 days <br />0.0108 hours <br />6.448413e-5 weeks <br />1.48395e-5 months <br />. This finding is of very low safety significance because there was no actual loss of safety system. This event is documented in CR 1702023
