Page3Testingofthe"B"EDGSupplyBreakertoBus16wasperformedinNovember,1997,duringthe1997outage.'Duringthistesting,thebreakerfailedtoclose.Thebreakercouldnotbeclosedelectricallyormanually,becausetheshunttripcoilplungerwasmaintainingthetripperbarinthetripfreeposition.Thisfailurewouldpreventthe"B"EDGfromsupplyingpowertothesafeguardsloadsonBus16.Thisisconsideredamajordegradationsince,inconjunctionwithasinglefailureontheredundanttrain,requiredsafetyfunctionscouldnotbeperformed.THEDATEONWHICHTHEINFORMATIONOFSUCHDEFECTORFAILURETOCOMPLYWASOBTAINED:TheinformationwasobtainedduringtestingperformedonNovember14,1997.INTHECASEOFABASICCOMPONENTWHICHCONTAINSADEFECTORFAILSTOCOMPLY,THENUMBERANDLOCATIONOFALLSUCHCOMPONENTSINUSEAT,SUPPLIEDFOR,ORBEINGSUPPLIEDFORGINNASTATION:TherearesixWestinghouseDB-75breakerspermanentlyinstalledatGinnaStation,insafeguardsbuses14,and16.Oneofthesebreakersismanuallyoperatedandtheshunttripcoilisnotused.Inaddition,thereisonespareDB-75breakerinstock.Withtheexceptionofthe52/EG1B1breaker,thesebreakersstillhavetheiroriginalshunttripassembliesinstalled.The52/EG181breakeristheonlyDB-75breakerthathashadtheshunttripassemblyreplaced.Thereplacementshunttripassemblythatfailedwasoneoftenpurchasedin1977asspares.THECORRECTIVEACTIONWHICHHASBEEN,ISBEING,ORWILLBETAKEN;THENAMEOFTHEINDIVIDUALORORGANIZATIONRESPONSIBLEFORTHEACTION;ANDTHELENGTHOFTIMETHATHASBEENORWILLBETAKENTOCOMPLETETHEACTION:Correctiveactioniscomplete.Thefailedshunttripassemblywasreplacedwithasparefromstock.Theassemblywasmanuallyandelectricallybench'testedpriortobeingreturnedtooperablestatus.Theremainingeightassembliesinstockwereinspected.Twoassemblieshadcoilplungersthatdidnotfullyreturntothedeenergizedpositionduringmanualmanipulation.Thesewereremovedfromstock.1TheotherDB-75breakerswerevisuallyinspected.Acceptableplungermovementisindicatedbyadequateplungertotripperbargapandfullretractionofthealarmswitchattachmentpushrod.Noadditionalbindingconcernswerefound.

Page4TheProcurementAnalysisForm(PAF)fortheshunttripassemblywasappended.Priortoreordering,thePAFnowrequirestheadditionofplungermovementverificationtotheassemblyacceptanceplan.Westinghousewascontacted.Therehavebeennootherreportedeventsofthisnature'thatWestinghouseisawareof.Acheckofindustryoperationaleventsdidnotidentifysimilarbreakerfailure.ANYADVICERELATEDTOTHEDEFECTORFAILURETOCOMPLYABOUTTHEFACILITY,ACTIVITY,ORBASICCOMPONENTTHATHASBEEN,ISBEING,ORWILLBEGIVENTOPURCHASERSORLICENSEES:Receiptinspectioncouldverifyacceptableplungermovement.DuringDB-75breakermaintenance,considerverifyingthattheshunttripcoilplungermovesfreely.inthecoilbyverifyingadequateplungertotripperbargapandfullretractionofthealarmswitchattachmentpushrod.
