Pagy-2-NMP-19742NiagaraMohawkresponse:Inourreviewofthisviolation,weconcurthatthecausewastheinadequatecontaminationsurveyperformedpriortopermittingflappingoperationsonf115RecirculationLoopBypassvalve,thoughRadiationProtectionProcedureS-RP-3providesadequateinstructions.Asaresultofthis,thefollowingactionshavebeentakentopreventrecurrenceofanincidentofthisnature:AformalmemorandumwasissuedtoallUnitIRadiationProtectionTechni-cianson5/21/86describingthesurveyrequirementscontainedinS-RP-3relativetoinsuringadequateevaluationofcontaminatedsurfacespriortopermittingflappingorsimilaroperations.Inaddition,thememoran-dumprovidedinstructionsrelatedtodecontaminationactivities,fixedcontaminationassessmentmethods,criteriaforrequiringrespirators,andtheproperuseofengineeringcontrols.ThismemohasbeenreadandunderstoodbyalloftheaboveindicatedtechniciansinaccordancewithRadiationProtectionInstructionRPI-1.2.On5/21/86,aRadiologicalIncidentReport(RIR-21)wasissuedtosum-marizetheinvestigationofthisincidentincludingappropriatemeasurestopreventrecurrence.ThisRIRwascompletedon5/23/86.3.On5/23/86,RadiationProtectionInstructionRPI-1,"InHouseRadiationProtectionTechnicianReadingAssignmentsandTraining",wasrevisedtorequireChiefandBackshiftRadiationProtectionTechnicianstoread,understandandinitialthe"RPSupervisorLogBook"priortobeginningactivitiesonatourofduty.4.Thecontractortechnicianresponsiblefortheradiologicalcontrolofthisflappingoperationfailed'tofollowapprovedproceduresthatspecifysurveyrequirementsandconditionsrequiringtheuseofeachtypeofrespirator.Asacorrectivemeasure,thetechnicianwasdismissedfromthesiteandplacedon2yearprobationbyhisemployer.NoticeofViolationItem250-220/86-08-01)TheInspectionReportstaes:"10CFR19.12requiresinpart,thatallindividuals,workinginorfrequent-inganyportionofarestrictedareabeinstructedinprecautionsandpro-cedurestominimizeexposureandthepurposeandfunctionofprotectivedevicesemployed.Contrarytotheabove,onApril28,1986,twoworkers,performinggrindingandlappingoperationsinpreparationforreplacingreactorwaterclean-up'uctionvalve33-02(highlyradioactivelycontaminated),wereprovidedinadequateinstructionsfortheinstallation:anduseofaglovebag.Asaresult,airtoolswereusedwithinthe'ag."'Air'exhaustingintothebagcausedthebagtolose,itsintegritytherebysubjectingtheworkerstoairborneradioactivityconcentrationsofabout800.timestheapplicable10CFR20concentrationvalues.In.addition,-andasa.result,oneoftheworkerssustaineda,limitedunplannedintakeofairborneradioactivematerial."
