The purpose of this meeting is to discuss the significance, cause, and corrective action associated with a preliminary finding with low-to-moderate safety significance (White). This finding involves the failure to use effective engineering controls as part of pre-job planning to reduce contamination and subsequent exposure of workers during Refueling Outage 16. The preliminary finding is discussed in NRC Inspection Report 05000382/2011009 (ADAMS ML112360693).
