STANDARD ANNUAL REPORT ON PRODUCTION FACILITY CHANCES, TESTS AND EXPERIMENTS AS REQUIRED BY 10 CFR50.59b Facility Changes MAR 76-9-3, completed 9/6/77 Replacement of thermomagnetic circuit breakers in the 3A1, 3A2, 3B1, 3H2 and 3AB ES MCC's with equivalent breakers rated at 50*C.
Original breakers were rated at 40*C, however, Requirement Outline permits up to the 50*C rating.
MAR 76-10-1, completed 9/6/77 Fabricate and install valve stem in MUV-49 (Letdown Cooler Isolation Valve). -Original stem is bent and replacement part is not readily-available.
Fabricated stem is from originally specified material and necessary material makeup was verified.
MAR 76-10-10, completed 9/6/77 Cut 1" controlled bleedoff piping of RCP-3A2 and add flange connections in order to expedite the removal and installation of RCP-3A2. This modification was designed and constructed to original codes and is for ease of maintenance only.
MAR 76-10-13, completed 10/14/77 Cut NSCCC piping and. install permanent flange connections in order to expedite the removal of RCP-3A2. This modification was designed and constructed to original construction codes and is ' for ease of mainten-ance only.
MAR 76-11-3, completed 10/14/77 Installation of 1/2" tubing to existing double isolated pressure taps of the RC system in order to obtain Reactor Coolant Pump differential pres-sure during pump operations. Failure of tubing would not cause collant leakage in excess of makeup capabilities.
MAR 76-11-9, completed 9/6/77 Installation of metal to metal seat conversion kits for specified solenoid valves in the CA, MU, and CF systems and lower disc stem clearance check of nine valves in the RM system. This modification is in r,esponce to NRC-IEB 75-03.
MAR 76-11-13, completed 10/14/77 Rotate the orientation of BSV-3 and BSV-4 so that the inlet and outlet are reversed in order to eleminate cavitation which' occurs at flows lower than 1350 gpm. This modification improves _the operation and effectiveness of the system.
i MAR 76-11-16, completed 9/6/77-Replace 3.3K resistor with 6.8K resistor in the remote meter assembly of RMG-19 in oefer to get remote indication to agree with local indi-ca t ion.'.This moolfication ensures accurate indication in the control room.
MAR 76-11-17,- completed 9/6/77 Revise setpoints of P & S 55, 68, 69, EP & ES-1, 5 and 9 in order to achieve proper temperature levels. Revise temperature alarm setpoints of IITRT-1. through 6 to avoid nuisance alarms. Replace defective equip-ment for EP & ES 18.
This modification does not reduce ~the effect or operation of the equipment.
MAR 76-11-23, completed 10/14/77 Reorient or extend reservoirs on 4 and 5 inch bore snubbers of all FPC piping systems in order to correct for a situation where reservoirs are slightly lower than snubber cylinders. This corrects an undesirable situation and allows units to function as designed.
MAR 76-12-6, completed 9/6/77 Installation of corrosive resistatt bushings in the solenoid linkage of RCV-10 (pressurizer electro-magnetic relief valve) in order to ensure freedom of movement and to decrease the probability of equipment malfunc-tion.
MAR 76-12-12, completed 9/6/77 Remove snubbers from RC pressure transmitters RC-3B-PT 1 through 3; RC-3A-PT 1 through 4, RC-131/132-PT in order to improve the response of these transmitters. There are no requirements for snubbers on RC pressure trans-mitters and the sensitivity of the transmitter is improved with these devices removed.
MAR 76-12-21, completed 12/2/77 Fabrication and installation of a support bracket on vertical run of pipe from discharge of MUV-137 between makeup tank and MUV-64 at an elevation of 96 feet.
Installation of the support bracket improves the desigh of the system by reducing the chance of line damage or breakage.

MAR 76-12-22, completed 10/18/77 Add a key lock selectable low range setpoint for operation of RCV-10 (pressurizer electromagnetic relief valve). This will prevent accidental overpressurization of RC system above NDT limits during plant heatup and cooldown.
MAR 76-12-22A To revise externa 1' connections of NNI system wiring to be compatible with internal.B & W wire, in order to connect interface as shown on original MAR 76-12-22.
This action was taken in response to NRC investigation of a generic problem of overpressurization of the RC system during startup and shutdown operations.
m MAR 76-12-25, completed 9/6/77 Add. flanges to'the waste disposal vent line.between DHP-B and DWV-30 and to the gland seal water line between the pump and Di[V-100 to allow for ease of removal of DHP-B.
The modification is built to original
- construction codes and will not degrade the operation and safety of
-the system.
MAR 76-12-27, completed 9/6/77 Notification of 'an Indication in the shaf t keyway on DilP-B and a crack in the carbon face of the mechanical seal.
It was determined-that pump operability was not in jeopardy. The unit was reassembled and put hack in service until a new replacement shaft was received and installed at a later date.

Change feed for Channel 2A cabinet on CAI print 208-028SE ESA-21 from VBDP 4 BKR 1 to VBDP 5 BKR 1.
This corrects an error on this print which reflects actual plant design conditions.
MAR 76-12-29, completed 9/6/77 Delete low range flow control on BSV 3 and 4.
Flow control in recir-culation mode with DHV-42/43 open will be an operator responsibility.
This modification permits the operator to have some control without reducing the capabilities and operation of the Building Spray System.
MAR 76-12-37, completed 9/6/77 Replace existing heat trace cable of sections EP and ES 3c, 4c, 11c and 12c in order to provide for lower temperatures on these circuits.
This modification will improve the operation of these systems.
}MR 77-1-8, completed 9/6/77 Replace the 1 1/2" air connection at Reactor Building penetration #341,.
spool piece B.S.67 with 8" pipe. A through wall crack was found in the heat effected zone. Alternate methods of conducting the 5 year flow tests for the Building Spray System exist. The integrity and safety of the system is increased.
MAR 77-1-17, completed 11/8/77 Install 1 1/2" diaphram valve in the line between WDT-9 and strainer WDS-1 and increase mesh size on WDS-1 'to 3/16" or 1/4".
This permits the removal of WDS-1 for cleaning without draining WDT-9.
The mesh size reduces cleaning requirements. The operation and integrity of system is not reduced.
MAR 77-1-24, completed-10/14/77 Install 1/2" Whitey valves in the leakoff piping of specified valves of the Dit & MU systems.
In the event that valve packings blow from any of these valves, system leakage will be contained by these insolation valves.

MAR 77-1-27,' completed 10/17/77 Add Concentrated 7aste and Boric Acid Return lines.to the Miscellaneous
. Waste and RC evaporators-and-install diaphram valves from spare parts

.the ability to reprocess low concentration tank inventory. The normal

operation of the system is not effected while its flexibility.is incredsed.
MAR 77-2-10, completed 10/17/77 Install a 1.38" orifice in the 3" flanges of NSCCC System at inlet to CRD coolers and revise calibration of SU-90 and 91-FT.
This modification reduces the load on the NSCCC pumps and will increase its lift. Proper cooling of the CRD's is monitored by outlet coolant temperature indication and alarms.
MAR 77-2-30, completed 10/14/77 Change setpoints of DO-47/48-PS from 25 psig to 15 psig in order to elim-inate the DO pumps drawing suction from the SW system rather than D0 tank.

This permits the system to operate as originally designed.
MAR 77-3-9,-completed 11/14/77 Replace Westinghouse Pressure Transmitters type 59PH with 'Rosemont Trans-mitters per a finding of "significant deficiency" by B & W.
Effected transmitters are RC-3A-PT 1 and 2, RC-3B-PT 1 and 2 and RC-132-PT.
The replacement of these pressure transmitters upgrade the installed system to meet the original design requirements.
I MAR 77-4-6, completed 9/6/77 Modify Steam Hammer Restraints MSH-227 and MSH-232 as per CAI redesign due to concrete pedestal cracking during testing of MS System (TP-800-25). The redesign will increase the safety of the system by insuring required re-straint capabilities.
l l-MAR 77-5-9, completed 10/17/77 l
Change the power feed for the Load Control Telemetry Cabinet from Vital Bus 3B-panel VBDP-A to Regulated Instrument Bus 3B-panel VBDP-2 in order to' resolve an underfrequency situation due to problems with Vital Bus 3B.
Both panels were designed for the additional capacity. Operation of the equipment will not be affected.
MAR 77-6-17, completed 10/17/77 Revise Elementary Diagram B-208-032, FW-47 revision 5 to correct for an incorrect terminal point reference. Drawing change only.
MAR 77-8-24, completed 11/14/77 Add calibrating resistors to Rosemont Pressure Transmitters RC-3A-PT 1 and 2, RC-3B-PT 1 and 2 and RC-132-PT to facilitate field calibration. These resistors aid in the calibration of the pressure transmitters but do not effect their operation.
