steel structures were found not to meet the requirements of the design drawing or the inspection procedure, QAP M-21.

Initial Report:

On' Janua ry 25, 1982, J Bryant of Region ll, Atlanta, Georgia was notified of the deficiency by W 0 Henry and J H Lanier of Duke Power Company, 422 South Church Street, Charlotte, NC 28242. (CA-82-09)

Description of Deficiency:

During the ba'ck-fi t structural inspection of Mechanical Equipment Supports as committed to per NRC follow-up Item 413/79-03-02 under QA Procedure M-18,

" Inspection of Structural Steel Erection", all accessible welds are re-Inspected during the M-18 process to the requirements of QA Procedure N-21.

Recently some welds were identified that did not meet the requirements of QA Procedure M-21 and the design drawing.

An investigation of the condition revealed that the welds which were identified were made prior to Rev 4 of M-18, which added a signature on the M-18A for verification that all welds ^ were in-spected to M-21.

Prior to Rev 4 of M-18 there was not a required signature that all the welds were inspected.

s Analysis o Safety implication:

Welds on nuclear safety related structures that do not meet design requirements' could impair the safety function of the structure. To date no such impairment has been identified.

i Corrective Action:

Structures were identified for examination of welds as typical of welding j per SAP M-21 prior to implementation of Rev 4 of the QAP M-18.

The welds were inspected to M-21 requirements and indications noted were evaluated by Duke Power Design Engineering-Civil. Design-Civil concluded that welding Indications typical of ones identified would not impair the safety function of structures shown on Civil Design Drawings.

Reactor building frames erected prior to implementatior of Rev 4 of QAP M-18 are under the Design' repsonsibility of the Support / Restraint Section of Mechanical Design Engineering Division.

For work completed after revision 4 to QAP M-18 a verification signature is required on Form M-18A to assure that all welds have been inspected per QAP M-21.

A final report will be provided by September 1,1982.
