j Special Report SR-94-05 Unit i Vibration and Loose Parts Monitor Operability of Channel 3 Associated Technical Specification:

3.3.3.10 Related PIP:

1-M94-1620 Cause of Malfunction:

Channel 3 (EMD ME 9000) of the Unit i Vibration and Loose Parts Monitor was declared INOPERABLE by Operations on October 24,1994 at 0500 Hrs. (TSAIL entry Ml-094-01135) due to noise. WR 94043758 was written to troubleshoot the problem. IAE investigated and identified the pmblem, however, the system is located in a high radiation area and this channel is not required for plant operation.

Instmment Vendor:

V& LPM manufactured by Rockwell Intemational.

Generic Concems:

The V& LPM has a history of unreliable performance. Numerous sensor and cable problems are the main contributor to this unreliable performance. These concems are being addressed by the panial completion of NSM MG-12096, replacement of the entim system, which will be completed during.

1EOC10. Installation during iEOC9 was delayed because of vendor equipment delivery.

Compensatory Measures:

No compensatory measures are planned or required.

Operability Restoration:

Channel 3 of the V& LPM will be repaired under WR 94043758 or replaced by NSM MG-12096 during outage IEOC10.

Future Plans:

Both Unit I and Unit 2 V&LPMs will be replaced by new and enhanced analysis equipment. The senson; and cables will be replaced at the same time.
