Dear Mr. Crow:

In reference to our telephone conversation of July 1,1985, I request

that the expiration date of the above license be extended to January 1,-

V 1986.

I stated in my June 28, 1985 letter to you that I will be sending a revised application for the license renewal for your approval by Novem-ber 29, 1985.

This additional time is needed to reformat our previous renewal application to conform with the guidance given in Regulatory Guide 3.52.

I am attaching a check in the amount of $150.00 to cover the fee associated with this request.
