E11GLOSURE 1 NOTICE OF VIOLATION Entergy Operations, Inc.                                            Docket No.:        50-458 River Bend Station                                                  License No.:      NPF 47 During an NRC inspection conducted on October 12 through November 29,1997, one violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG 1600, the violation is listad below:
Technical Specification Surveillance Requirement 3.0.1 states, in part, " failure to meet a Surveillance, whether such failure is experienced during the performance of the Surveillance or between performances of the Surveillance, shall be failure to meet the LCO "
Technical Specification Surveillance Requirement 3.6.1.2.1 states, in part, " Perform required primary containment air lock leakage rate testing in accordance with the Primary Containment Leakage Rate Testing Program."
Technical Specification 3.6.1.2, Condition A, applies to containment air locks with one door inoperable. Technical Specification 3.6.1.2, Condition B, applies to containment air locks with the primary containment air lock interlock mechanism inoperable.
Technical Specification 3.6.1.2, Condition C, requires, in part, with cne or more primary containment air locks inoperable for reesons other than Technical Specification 3.6.1.2, Conditions A or B, immediately initiate action to evaluate primary containment overall leakage rate per Technical Specification 3.6.1.1, using current air lock test results.
Contrary to the above, on October 30,1997, at 8 a.m., the lower primary containment air lock failed during the performance of Technical Specification Surveillance Requirement 3.0.1.2.1, thus rendering the air lock inoperable for reasons other than Technical Specification 3.6.1.2, Conditions A or B, and action to evaluate primary containment overall leakage rate per Technical Specification 3.6.1.1, using current air lock test results, was not completed.
This is a Severity Level IV violation (Supplement l} (50-458/9717-04).
Pursuant to the provisions of 10 CFR 2.201, Entergy Operations, Inc. is hereby required to submit a written statement or expl.anation to the U.S. Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Rogion IV,611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy to the NRC Resident inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting th s Notice of Violation (Notice). This reply should be clearly marked as a
    " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further v5lations, and (4) the date when full compliance will be achieved.
