Dear Mr. Winslow:

This refers to Certificate of Compliance No. 9019 for the Model No. BU-7 packaging which has an expiration date of February 28, 1993.

Since the certificate references include several supplements, a consolidated application, complete in itself and which includes drawings and all pertinent information should be submitted. Preferably, the application should be in the format of Regulatory Guide 7.9. The sections on operating procedures, acceptance .

tests, and maintenance programs should be reviewed to assure they are complete 1 and current. If revisions are necessary, the revised sections should. be submitted with the application for renewal. Note that the certificate- of compliance may be conditioned -to specifically cite the operating procedures, acceptance testing and maintenance programs.

This notice of expiration should not be construed .that such notice will be provided in the future. Also, please let us know if you do not intend to reques':
