All Refuel 14 required Pressure Tests for Category B-P (8 are required to be performed every Refuel Outage) were completed. The B-N-1 component exam was performed during the RF-12 Outage (Period 2) and credited for both Cycle 12 OAR-1 and Cycle 13 OAR-i. The Cycle 14 OAR-1 will show that the 3rd period has not been credited with the B-N-1 exam and that the interval percentage stayed the same as the Cycle 12 OAR-1 interval percentage. This exam is scheduled for examination in Cycle 15 (Period 3).

Submittal of Owner's Activity Report Form for Inservice Inspection Performed During Refueling Outage 14 Waterford Steam Electric Station, Unit 3 Docket No. 50-382 License No. NPF-38

Dear Sir or Madam:

Attached is the subject report which covers the Owners Activity Report Form, OAR-i, for Waterford 3 refueling outage (RFO) 14, enclosed as Attachment 1. This report is being submitted in accordance with ASME Code Case N-532-1, "Alternative Requirements to Repair and Replacement Documentation Requirements and Inservice Summary Report Preparation and Submission as Required by IWA-4000 and IWA-6000," which was adopted for use at Waterford 3 commencing at the end of RFO-1 2.

The OAR-1 form is prepared and certified upon completion of each refueling outage. This OAR-1 form provides a summary of the examinations performed during the refueling outage in the third period of the second 10 year Inservice Inspection (ISI) interval. A copy of this letter and the attached report will also be submitted to the office of the Louisiana State Fire Marshall.

Inspections were performed in accordance with Technical Specification 3/4.4.4 "Steam Generator (SG) Tube Integrity" and NEI 97-06 "S/G Program Guidelines". Waterford 3 performs 100% Eddy Current inspection of the inservice Steam Generator tubes every Refuel Outage.

Note 3:

Per 10CFR 50.55a(b)(2)(ix)(E), a general visual examination is required for each period.

This examination covers all accessible pressure retaining surfaces. A general visual examination was completed for all accessible surfaces in this period:

There are currently no exam category E-C Examinations required at Waterford 3.

Note 5:

100% of the moisture barriers are examined due to detected flaws. 33% of the moisture barriers are credited for the 3 rd period requirements.

Note 6:

Currently 89 C-H examinations selected for each period from a total population of 89.

Currently 59 D-B examinations selected for each period from a total population of 59.
