Diesel Generators Inoperable CONDITIONS PRIOR TO OCCURRENCE:
Mode 6 DESCRIPTION OF OCCURRENCE:
The llSW nuclear HDR was out of service and cleared and tagged for repairs. The diesels were found to be overheating. All the diesels were declared inoperable and Action Statement 3.8.l.2b was entered at 2158 hours0.025 days <br />0.599 hours <br />0.00357 weeks <br />8.21119e-4 months <br /> on October 8, 1980. It was determined that when the indicator for the 12SW21 valve was in the open position, the valve was actually closed. The valve was opened and tagged to remain in th open position by the shift supervisor until the indicator was properly aligned.
It was determined that sufficient cooling water was available for the diesels. The lA Diesel was run for one hour with no overheating problems and subsequently declared operable. Then the lB and lC Diesels were run at the same time for an hour with no overheating problems and subsequently declared operable. The Action Statement
.was terminated at 0216 hours0.0025 days <br />0.06 hours <br />3.571429e-4 weeks <br />8.2188e-5 months <br /> on October 9, 1980.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The apparent cause of the occurrence was that the valve positioner and indicator were aligned incorrectly with the valve.
ANALYSIS OF OCCURRENCE:
Inoperable diesel generators constitute a loss of an emergency power source.
CORRECTIVE ACTION:
The 12SW21 valve positioner and indicator were aligned correctly.
