Thesemodifications requiredthattheaffectedsideoftheFireDetection circuitry (either"A"or"B")betakenoutofservice.Technical Specification 3.3.3.7requiresthatafirewatchbeestablished andafirepatrolbeperformed every8hoursinthecontainment shieldbuildingannulusareawhiletheassociated firedetectors areoutofservice.OnMay15at2230,an8hourfirewatchpatrolwasperformed ontheshieldbuildingannulus.At0100onMay16,Operations beganthemonthly10hoursurveillance runofthe6BShieldBuildingExhaustfan(EIIS:VG),
perTechSpec3.6.6.1.At0630,contractor maintenance personnel onthefirewatchteammadeanattempttoaccesstheannulustoperformthefirewatch,buttheannulusdoorcouldnotbeopenedduetothedifferential pressurecreatedbythefan.Thecontrolroomwasnotified, buttheutilitylicensedseniorreactoroperatorassumedthatthefirepatrolwasrequiredshiftly(Technical Specifications define"shiftly" requirements asthosetobeperformed every12hours),ratherthanevery8hours,anddidnotverifyhisassumption againsttheTechnical Specifications.
At0840,thecontractor notifiedtheutilityFireProtection department thatthefirewatchwasnotperformed.
Atthattime,thecontrolroomwascontacted andthe6BShieldBuildingExhaustfanwassecured.Theannulusfirewatchwasperformed at0913,afteraperiodof10hours,43minutes.Therootcauseofthiseventwaspersonnel errorbyautilitylicensedSeniorReactorOperator.
Therequ'lred 8hourfirewatchoftheannulusareawasdelayedbytheconcurrent performance ofthe10hourmonthlysurveillance runofthe6BShieldBuildingExhaustfan.Thedifferential pressureacrosstheannulusdoorwassuchthatitwasphysically impossible toopenthedoor.Additionally, whentheUnit1SeniorReactorOperatorwasnotifiedoftheproblem,heincorrectly assumedthatthefirewatchwasashiftly(12hour)requirement, ratherthanan8hourrequirement.
Therewerenounusualcharacteristics oftheworklocationthatcontributed totheerror,norwereanyactionstakencontrarytoanyapprovedplantprocedures.

Thisspecification requiresaminimumof1operabledetectorinzoneBoftheannulusregion.Withthe"B"sideFireDetection systemtakenoutofservice,nofiredetectors wereavailable inzoneBoftheannulus,andthus,an8hourfirewatchwasestablished.
BothzoneAdetectors wereoperablethroughout theentireevent.ThezoneAandzoneBdetectors occupythesamegeneralareaandarenotseperated byanyphysicalbarriers; thusfiredetection intheannuluswas,available throughout thistimeperiod.Thetotalelapsedtimebetweenfirepatrolswas10hoursand43minutes,whichonlyslightlyexceededthe8hourintervalrequiredbyTechSpecs.Thehealthandsafetyofthepublicwasnotendangered atanytimeduringthistimeperiod.1)Operations securedthe6BShieldBuildingExhaustfantoallowaccesstotheannulusforthefirewatch.2)Thefirewatchenteredtheannulusandperformed therequiredfirewatchpatrol.3)The6BShieldBuildingExhaustfansurveillance wasdeferreduntiltheBsidefiredetection systemwasplacedbackinservice.4)Trainingwillreviewthiseventforpossibleincorporation intotheLicensedOperatorRequalification Trainingprogram.5)ControlRoomsupervision willreceivecounselling oncorrectinterpretation ofTechnical Specification firewatchrequirements.
