The inspector found that the procedure contained appropriate guidance to preclude challenges to the plant associated with the use of freeze seals.

.However, this failure constitutes a violation of minor significance and is being treated as a Non-Cited Violation. consistent with Section IV of the NRC Enforcement Policy (NCV 335/96-11-03. " Failure To Properly Monitor Freeze Seal") The inspector reviewed the control room logs and noted that there were no log entries which would indicate that the freeze seals were left unattended.

In aodition. the inspector verified that the thaw time of each of these seals would have been on the order of two to three hours and therefore concluded that'the safety impact was minimal.

While observing Maintenance personnel disassemble the V2167, the inspector noted that the tools and equipment needed to support the job had been laid out and arranged on tne floor.

This was discussed with the personnel performing the task who stated that this was to aid in identification while performing the work and wa.s a new management initiative to improve job performance.

The inspector discussed this with the Maintenance Manager who concurred and stated that this practice along with generally maintaining a clean work area. being prepared to do the work prior.to starting, etc.. would decrease rework and ultimately improve efficiency.

The inspector also discussed this with the Health Physics suoervisor who stated that, with the efficiency increases, overall dose expended in maintenance was expected to drop.

The inspector concluded that these efforts were prudent and that benefit could be attained from them.
 
CONCLUSIONS:
-The inspector was able to determine that a maintenance manager did shut down all jobs in the pice tunnel on May 22. 1996.
 
The inspector concluded that the concern was partially substantiated in.that the person monitoring the freeze-seal was in the area, but he was not actively monitoring the seal as evidenced by the absence of freeze seal temperature data for the time in question.

result. while people were available to implement a contingency plan in the event of_ problem with the freeze plug.- for a time the freeze seal temperature was not being monitored. -The inspector determined that this was a violation of the licensee's crocedure.which requires that at no time shall a freeze seal

Crystal River.- Failure to perform an adequate 10 CFR 50.59 evaluation on diesel generator loading following a modification to remove the automatic-start of the turbine-driven auxiliary feedwater pump.

St. Lucie -
The Maintenance Rule Inspection was conducted during the week. Three violations were. identified. The violations identified were in the areas of (1) program design issues: (2) system scoping issues: and (3)
procedure implementation.
 
The residents are continuing to inspect a drop control rod event in which the licensee installed the wrong control card as part of the corrective action.
 
The residents are also inspecting labeling problems and the independent verification process following the bypassing of an incorrect bistable.

Son Ninh was approved for transfer from NRR to RII as a project engineer

for Branch 3 assigned to FPL.

Repairs were completed for the turbine lubricating oil system leak and reenforcing the reactor building sump screens.
 
The turbine lubricating
,
oil pipe ruptured due to hydrogen detonation.
 
The hydrogen is entrained in the oil from interfacing with the hydrogen cooling system and collected in the high point where the rupture occurred. The check valve appears to have provided the spark for the detonation.
 
Leakoff holes have been drilled in the top of the lube oil pipe to preclude the collection of hy,drogen.
The apparent tampering event associated with the "A" EDG is still under investigation by the licensee, c)
Turkey Point - Unit 4 is operating at 100%.
Unit 3 is in mode 2 continuing the startup following repair to of rod control circuit cards which led to two control rods dropping (see the attached event report)

Billy Crowley will review the report on 10/7/96 and conduct a public exit on 10/9/96 at 9:00 am.

Crystal River - Emergency Feedwater (EFW) susceptibility to single failure: during LOOP a failure of one fuse could cause an EFW flow control valve to fail-open allowing one EFW pump to go to runout.
 
Since both EFW l
pumps draw suction through the same pipe, the resulting flowrate would cause cavitation and failure of both EFW pumps.
 
b)
St. Lucie -
The Maintenance Rule Inspection identified three potential violations which will be reviewed for an enforcement panel on 10/7/96.

S.t Lucie - Unit 2 operated at 100% for the entire week. Unit 1 operated at 100% power until 10/17/96, then reduced power to 78% to repair a main condenser water box.

Turkey Point - Both units are operating at 100%.
Unit 3 completed its power uprate from 2200 to 2300 MWth on 10/11.
 
Unit 4 uprate is scheduled for next week.
 
Region 11 is monitoring the progress of hurricane Lili and the licensee's preparations for the hurricane (see attached PN).
 
c)
Crystal River - The unit remains shutdown pending a license amendment.to resolve several US0s on EDG loading and EFW susceptibility to single failure.
 
FPC plans to remain shutdown for another two months or more to resolve the design issues and submit the necessary license amendments.
 
Emergency Preparedness Exercise was conducted on. 10/16/96. no major deficiencies were identified.

Crystal--River - Failure to perforril an adequate 10 CFR 50.59 evaluation on diesel generator loading following a modification to remove the automatic. start of the turbine driven auxiliary feedwater pump.

Crystal River EFW susceptibility to single failure: during LOOP'a failure of one fuse could cause an EFW flow control valve to fail.open allowing one EFW pump to go to runoute Since both EFW pumps draw suction through the same pipe, the resulting flowrate would cause cavitation and failure of both EFW. pumps.

-St Lucie/ Turkey Point - A vulnerability was identified in the safeguards process for FPL where the access authorization was not immediately removed for individuals whose Eemployment was terminated. One individual after employment termination reentered the site _ approximately five time for a total of 15 hours.
