              .' nit 2 Technical Specification 6.8.1 requires that written procedures be established, implemented and maintained covering the activities recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978. Item 3.d of this Appendix requires procedures for the operation of the emergency' core cooling system.
Procedure 2104.39, "HPSI System Operational, has been establisheo in accordance with this Technical Specification requirement.
Attachment A. "A SIAS Standby," of this procedure requires that Manual Valve 2SI-1042A be normally closed.      Supplement I, "2P-89A Monthly Test," of this procedure also requires that discharge test gauges be isolated at the completion of the monthly high pressure injection pump test by closing Valve 2SI-1042A.
Contrary to the above on July 14, 1988, the NRC inspector found that Manual Valve 2SI-1042A was open.
This is a Severity Level V violation.        (Supplement I) (368/8823-01)
Response to Notice of Violation 368/8823-01 (1) The reason for the violation if admitted:
AP&L admits the violation as stated above. The reason for the violation was procedural inadequacy. When performing monthly testing of high pressure iniectior, pump 2P-89A, a permanently mounted test gauge, 2PI-5091, is used to obtain pump discharge pressure. Other than for monthly testing, this gauge is normally isolated from the system piping by two valves 2SI-1042A and 2SI-1043A. The monthly testing procedure, Supplement I of Procedure 2104.39, only requires that the test gauge be isolated following testing. This requirement was not as explicit as the system lineup procedure, Attachment A of Procedure 2104.39, which requires specific operator verification of the closure of each isolation valve for the test gauge. As a result, following monthly testing, the gauge was isolated by closing only one valve, 2SI-1043A. and not both valves (2SI-1042A remained open) contrary to the system lineup requirements.
(2) The corrective steps which have been taken and the results achieved:
Valve 2SI-1042A was closed on July 14, 1988, after the NRC inspector informed Operations that it was found open. As an interim corrective action, a memorandum was issued from the ANO-2 Operations Superintendent to all Unit 2 operators which      ,
directs the opt"ators to close both isolation valves for all      '
safety-related pump gauges to isolate a gauge following pump surveillance testing. ANO-1 typically utilizes temporary gauges    ,
which are installed and removed following testing. Permanent gauges are being installed during the current ANO-1 refueling outage.
