This refers to the inspection conducted from March 21 to May 1,1999, at the Beaver Valley | |||
Power Station facility. The enclosed report represents the results of this inspection. | |||
During the 6-week inspection period, your conduct of activities at the Beaver Valley Power | |||
Station facility was characterized by safe conduct of activities, especially during the Unit 2 | |||
refueling outage and the Unit 1 surveillance outage. Your staff's human performance improved | |||
as compared to the 1998 extended outages. This improvement was attributed to the strong | |||
response to lower safety significant problems at the beginning of the Unit 2 outage and initiatives | |||
such as the one-stop-shop outage work management concept. Surveillance procedure | |||
deficiencies continue to challenge your operating staff. However, surveillance testing and | |||
maintenance activities were generally performed well, in part, due to strong proevolution | |||
briefings.' Emergent issues were properly addressed with good root-cause assessment by | |||
system engineers and maintenance personnel. | |||
The radiation' protechon program was effective during the Unit 2 refueling outage. Enhanced | |||
informational postings, especially in the containment, aided in maintaining occupational | |||
exposures as low as is reasonably achievable (ALARA). The scope and depth of Quality | |||
Services Unit surveillances conducted for the outage, and as part of the annual health physics | |||
program audit, were adequate to identify and document program deficiencies. | |||
