Technical Specification 6.8.1.a requires that written procedures listed in Appendix A of Regulatory Guide 1.33, November 1972, be implemented. Item A.3 of Appendix A requires procedures for equipment control. Equipment control procedure 01-13, Step 4.5, requires all valves be maintained in position required for the OP valve lineup. OP-46 requires valves 2-RNA-V202 and V203 to be in the open position. x Contrary to the above, Technical Specification 6.8.1.a was not implemented in that valves 2-RNA-V202 and V203 were not maintained open on September 4, 1985. | |||

I. Admission or Denial of the Alleged Violation CP&L .scknowledges the violation occurred as stated. | |||
II. Reason For Violation Clearance No. 2-1107 necessitated closure and red tagging of Unit 2 Reactor Building Service Air (SA) System valves 2-SA-V301 and V302. 'IVo auxiliary operators (Aos) were dispatched to accomplish this task. The clearance tag sheet specified the location of V301 and V302 as in the overhead vicinity of the electrical motor control center 2XM. One of the A0s climbed to the described location and closed what he believed were the desired SA System valves. However, actually he closed isolation valves V202 e ar.d V203 of the Reactor Building Noninterruptible Instrument Air (RNA) to the control rod scram air header. | |||
An assessment of the circumstances leading to the closing of the RNA isolation valves revealed an error existed in the SA System operating procedure (OP) valve lineup sheet. The OP valve lineup sheet incorrectly referenced the location of the SA valves. The involved equipment clearance was written utilizing the subject valve lineup sheet along with appropriate plant drawings to describe on the clearance tag sheet the . | |||
