10 CFR 20.311(c) requires, in part, the manifest must include a certifi-cation by the waste generator that the transported materials are properly described. | |||
Contrary to the above, on February 19, 1988, your Shipment Number 88-025-II of radioactive waste was sent to a burial site and was accompanied by a manife;t which reported a different package than the one which was shipped. | |||
As a result, the total radioactivity stated on the manifest was in error. | |||
Further, the certification which accompanied the manifest was also in error. | |||

Contrary to the above, on February 19, 1988, your Shipment Number 88-025-II of radioactive waste was sent to a burial site and the shipping papers did not include the activities of the radionuclides contained in one package in the shipment. | |||
Violations A and R have been categorized in the aggregate as a Severity Level IV problem. | |||
