. Dear Mr. Cross:
This refers to the inspection conducted from March 21 to May 1,1999, at the Beaver Valley
Power Station facility. The enclosed report represents the results of this inspection.
During the 6-week inspection period, your conduct of activities at the Beaver Valley Power
Station facility was characterized by safe conduct of activities, especially during the Unit 2
refueling outage and the Unit 1 surveillance outage. Your staff's human performance improved
as compared to the 1998 extended outages. This improvement was attributed to the strong
response to lower safety significant problems at the beginning of the Unit 2 outage and initiatives
such as the one-stop-shop outage work management concept. Surveillance procedure
deficiencies continue to challenge your operating staff. However, surveillance testing and
maintenance activities were generally performed well, in part, due to strong proevolution
briefings.' Emergent issues were properly addressed with good root-cause assessment by
system engineers and maintenance personnel.
The radiation' protechon program was effective during the Unit 2 refueling outage. Enhanced
informational postings, especially in the containment, aided in maintaining occupational
exposures as low as is reasonably achievable (ALARA). The scope and depth of Quality
Services Unit surveillances conducted for the outage, and as part of the annual health physics
program audit, were adequate to identify and document program deficiencies.
