ENCLOSURE NOTICE OF VIOLATION Ryder Memorial Hospital License No. 52-21026-01 The following violation was identified during an inspection conducted on February 12, 1986. The Severity Level was assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).
License Condition 15 requires that except as specifically provided otherwise in this license, the licensee shall possess and use licensed material described in Items 6, 7, and 8 of this license in accordance with statements, representations, and procedures contained in application with ALARA Program and other enclosures dated March 30, 1982 and letters dated May 4, 1982 and November 30, 1983.
The Nuclear Regulatory Commission's regulations shall govern the licensees statements in applications or letters, unless the statements, representations and procedures in the licensee's application and correspondence are more restrictive than the regulatior.s.
Section 10 of the application requires that the dose calibrator be checked for linearity on a quarterly basis.
Contrary to the above, the dose calibrator was not checked for linearity between May 14, 1984 and November 26, 1984 and between May 6, 1985 and October 7, 1985. Both instances exceed the quarterly requirement.
