Review of the November 9, 1991, Salem Unit 2 Turbine-Generator Overspeed and Fire Event File: 92-029-404 92-056-026 The purpose of this letter is to inform you of the actions Arizona Public Service Company (APS) has completed and those planned to address the aspects of the Salem Unit 2 Turbine-Generator and Fire Event.

On November 11, 1991, APS received notification over the Nuclear Network which

-included preliminary information concerning the Salem event.

This information was distributed to management personnel at PVNGS via the PVNGS Tech News.

In December 1991, PVNGS formed a team to investigate this event for applicability to PVNGS.

This investigation was based on the information available from the Nuclear Network; The details of NRC Information Notice 91-83 were included in the investigation

Mr. John B. Martin U.S.'Nuclear Regulatory Commission PVNGS Review of the NRC Region I AIT Report on the Salem Turbine-Generator Event Page 2 when APS received the Information Notice in late December.

The investigative team considered system design and other factors that contributed to the Salem event.

The three documents available from the Institute of Nuclear Power Operations and the Information Notice on this event were added to the. required reading list for the Operations Department.

As an additional precaution, a pre-test briefing was conducted with all testing personnel prior to the January 9, 1992, overspeed test on Unit 2.

Prior to this test, the investigative team reviewed the PVNGS Turbine-Generator System configuration and testing procedures to ensure that adequate controls were in place to avoid similar events at PVNGS.

Upon receipt of the additional information included in the AIT report, the original review team (formed in December 1991) was expanded and charged with performing an in-depth review and application of lessons learned at PVNGS. The team is reviewing the event from a plant-wide perspective and willapply the lessons learned to the applicable plant systems, procedures, and programs.

The PVNGS review team members have identified the areas of system design, system operation, system testing, system trending, operations procedures, preventative maintenance, emergency planning, and work control as the scope of their review.

The preliminary review effort is complete, and the review team members are currently performing their in-depth reviews of assigned action area's.

Corrective actions identified as a result of this review willbe scheduled for implementation in a timely manner.
