Momentive Batch Number 14ELVS145 (iv} Nature of the defect or failure to comply and the safety hazard which is created or coutd be created by such defect or faiiure to comply.

PG&E notified CW l;nertech that they observed white particutate trt SF-1154 fluid in three containers. The whi1e particulate wa$ found settled at the bottom of the containws. The fluid was dedicated and supplied by CW Enertech in November 2016. The fluid was traced back to Momentive Batch 14ELVS145.

The Mor.nentlve Batch 14ELVS145 was previously reported by Lake Engineering Company (Ref.

NRC ML 17212Af528 and ML 17128Me5). The white *particufateslsemisotid material was identified as phenyl cyclic precipitate. The safety hazard that could be created by this defect fs the blockage of Snubber bleed port as reported t>y Duane Arnold (Re.f. ML070300154). This blockage coutd prevent the Snubber from unlocking after a s-e1smic eve.nt thus preventing the Sm1bber to alfow for

• system movement during normal operati.ons.

With all solids dissolved, there is no potential safety hazard with this fluid.

(v) The date on which the information of such ctefect or failure to comply was obtained.

PG&E notified CW Enertech of the defect on February 13., 2018: Three 5 gallon containers of fluid were returned to Enertech on RMA-4215 and received on March 22, 2019 tor evaluation.

(Yi) tn the case of a basic component whicli contains a defect or fails to comply, the rwrnber and tocatron of these components in use at, supplied for, being suppfied for, or may be supplied tor, manufacture, or belng manufactured for 011e or more facilities or activities subject to the regulations in this part.

CW Enertech procured three 55 gallon drums of Momentive Batch 14ELVS 145. The bafcti was

• separated into smaller 5 gallon containers and suppUed to the following sites:

Detroit Edison Company, Fermi Duke Energy/Progress Energy, Shearon Harris First Energy Corpotation, Beaver Valley Pacific Gas & Electric, Diablo Canyon Tennessee Valley Authority, Watts Bar Fir$t Energy Corporation, Perry Tecnatom, Almaraz NPP (vti} The cortective action which has been, is. being, or wiH be taken: the name.of the individual or organization responsible for the action: and the length of time that has been or wnl be taken to complete the action.
