The White finding involved the licensee's failure to establish measures for the selection, and review for suitability of application of materials, parts, equipment, and processes that are essential to the safety-related functions of the structures, systems, and components involving the Dresden 3 Automatic Depressurization System (ADS). Specifically, from November 29, 2010, to November 6, 2014, the licensee failed to establish measures to ensure that the application of the ADS electromatic relief valve (ERV) actuators remained suitable for operation. This resulted in multiple failures of the ERV 3E during testing prior to operating cycle D3C23, and an indeterminate period of inoperability and unavailability greater than allowed by Technical Specifications during operating cycle D3C23. The ERV 3E inoperability during the operating cycle was identified after the failure of the valve during its first operational test following unit shutdown for refueling. | |||
