This document addresses the reporting requirements for Surry Unit I forthe portion of the third period in the fourth ten-year ISI interval from November 21, 2013to October 13, 2014 and through the duration of the first refueling outage of the firstperiod in the fifth ten-year inservice inspection interval. In our December 9, 2013 letter(Serial No.13-639), Revised Relief Request SPT-003 stated that the Surry Unit I fourthISI interval was being extended to October 13, 2014, and our November 26, 2013 letter(Serial No.13-611) identified the fifth ISI interval start date as December 14, 2013.If you have any questions concerning this information,Mrs. Candee G.
