Licensee-Identified Violation

Description

The following violation of very low safety significance (Green) was identified by the licensee and is a violation of NRC requirements which meets the criteria of the NRC Enforcement Policy for being dispositioned as a non-cited violation. Fort Calhoun Station Technical Specification 5.8.1, requires, in part, that the licensee establish and implement written procedures recommended in Regulatory Guide 1.33, Revision 2, Appendix A, dated February 1978, including procedures for equipment control (e.g., locking and tagging). Contrary to this, containment integrigity was violated on November 1, 2009, when an inadequate procedural step in a surveillance test procedure required by Regulatory Guide 1.33 allowed opening of a locked closed containment isolation valve, thus violating containment integrity. The finding was determined to be of very low safety significance (Green) as it did not result in an actual release of radioactive material. Because this violation was of very low safety significance and it was entered into the licensees corrective action program as Condition Report 2010-1664, this violation is being treated as a non-cited violation consistent with Section 2.3.2.a of the NRC Enforcement Policy.
