2.0CONCLUSION

Theconclusionofthisevaluationisthatthelimitsofminimumcriticalpowerratio(MCPR),peakvesselandmainsteamlinepressures,andpeakfuelcladdingtemperaturefortheexpectedoperationaloccurrencecategoryoftheidentifiedeventswouldnotbeexceededasaresultofacommonsensorlinefailure.Althoughtransientcategoryeventshavebeenpostulatedasaresultofthisstudy,theneteffectshavebeendeterminedtobelessseverethanandboundedbytheeventsinChapter15.3.0ANAIYSISMETHODOLOGYInconjuctionwiththeNMP-2ControlSystemsCommonPowerSourceFailureAnalysis(CPA)portionoftheoverallNMP2ControlSystemsFailureAnalysisProgram,acomprehensiveapproachwasdevelopedandimplementedtoaddressthegeneralpurposeoftheanalysesaswellasthespecificNRCNMP2FSARQuestion421.42concerns.Theactivitylist,Table1.1,andfollowingdescriptionshighlightthemethodologyusedtoperformtheanalysis.Itshouldbenotedthatthisstudyusedtheevent-consequencelogicoftheNMP2FSARChapter15analysis,butitstartedthelogicchainfromaspecificsource(e.g.,asinglecommonsensorlinefailure)ratherthanasystemcondition(e.g.,feedwaterrunout).Byapproachingthestudyinthismanner,agreatdealofconfidencecanbeplacedinthestudyconclusions.Thesoundnessofthetotalplantdesignisdemonstratedbyitsbeingtolerantoftheseeffects.12-25342

3.1SystemIdentificationThescopeofcontrolsystemstobeanalyzedwasestablishedbyfirstcompilingacompletelistoftheNineSilePointNuclearStationUnit2systemsandsubsystems.Next,thelistwasreviewedtoconfinetheanalysistoonlythosesystemswhich,intheirnormal(automatic,ifavailable)controlmode,havethepotentialtoaffectreactorpressurevessel(RPV)pressure,waterlevel,orpowerlevelchanges.3.1.1AlltheHMP2plantinstrumentationandcontrolsystemswereidentified,listed,andagreeduponascompletebythetwoprincipalanalyzingengineergroups,i.e.,GeneralElectricCompany(GE)andStoneandWebsterEngineeringCorporation(SWEC)..3.1.2Systemandcomponenteliminationcriteria(seeTable1.4)were'derivedandagreeduponbytheprincipalstodeletenon-electrical,non-opera-tional,ornon-controlsystemsorcomponents(includingsomepreviouslyanalyzedsystemsandcomponentsalreadyaddressedinFSARChapter15)fromthesystemsidentifiedin3.1.1above(seeTables1.2and1.3).Iftherewas.anyuncertaintyastowhetherornotasystemmet,thecriteria,itwasretainedforfurtheranalysis.Thosesystemsthatmetthecriteriaforeliminationweresonotedinthecompletesystemlist,leavingtheremainingcontrolsystemstobeanalyzed.3.2CommmonSensorLineorSensorIdentificationTheCommonSensorFailuxeAnalysisportionoftheControlSystemsFailureAnalysesthenidentifiedstrategicreactorprocesssensorlinesorsensorscommonlysharedbytwoormoreplantsystems,atleastonesystemofwhichwasanon-safetygradesystemidentifiedinSection3.1.2above.3.3Failure@peDeterminationBasedonconservativeassumptions,acompleteandinstantaneoussensorlinebreakorplugduringnormal,fullpowerreactoroperationwasdetexminedtobetheboundingfailuretypesforeachsensorlineanalyzed.3.4DefinitionsECommonInstrumentLine:Alineprovidingaprocesspressuresignaltotwoormoreinstrumentsensors(pressuretoelectricalcurrentinstru-ment,P/I,transmitters)whichservetwoormoreinstrumentsystems,ofwhichatleastonesystemisanon-safetygradecontrolsystem,e.g.,FeedwaterControlSystem(C33).CommonInstrumentSensor:Aninstrumentsensorwhichprovidesinputstotwoormoreinstrumentsystems,ofwhichatleastonesystemisanon-safetygradesystem.12"25343

LineFailureTypes:Broken:Aninstantaneousinstrumentlinebreak(guillotinebreak)thatventstoanambientpressure(nearatmosphericpressure)environment.Plugged:Aninstantaneousinstrumentlineplug(completeblockage,pinch)maintainingas-failedlinepressureattheinstrumentsensorandessentiallyinhibitinganymonitoringchange,especiallyactualprocess,vessel,orlineparameterchanges.Note:Inthecaseofdifferentialpressuresensinginstrumentsmonitoringreactorpressurevesselwaterlevel,apluggedreferenceorvariablelinecouldresultin,amorecomplexresponse.Theconservativeresponse,however,isstillaninhibitedresponse,i.e.,noactionswouldoccur.PrimaryEffects:Thedirect,instantaneouseffects,ifany,onthespecificallyidentifiedsensororcomponentresultingfromthefailure.Usually,thesensedinputsignaltothesensorcomponentgoestoaminimumormaximumvalueor,inthecaseofasensorlineplug,remainsrelativelyconstantataninaccurate(as-failed)value,insensitivetoanyactualprocesschanges.SecondaryEffects:Theindirecteffect,instantaneousordelayed,ifany,onthespecificallyidentifiedsensororprominentsubsequentinstrumentloopcomponents,i.e.,indicators,tripunits(trips,permis-sives,initiators),controls{controllers,valves),ordevices(relaysorlights).RPVLiquidLevelPressureorPowerLevelEffects:Anyactualorpro-bablereactorpressurevesselliquidlevel,pressureorpowerlevelchangedirectlyorindirectlyattributabletotheidentifiedfailureandcomponentactionsorinactions.ICombinedEffects:Thesystematicevaluationoftheidentifiedlinesandsensorsprimaryandsecondaryeffectsandtheresultingaction(s),ifany,whichwouldmostlikelyresultasadirectaccumulationofeach,orall,sensorfailuresandRPVpressure,liquid,andpowerlevelchangeeffectsonplantperformance.3.5Line-GroupandComponentTabulationandFailureAnalysisThesensorinstrumentationdirectlyconnectedtoandreceivinganinputsignalfromtheidentifiedsensinglineorsensorwereindividuallyidentified,grouped,listed,andanalyzedtodeterminewhat,ifany,actionwouldresultfromtheoccurrenceofeachline/sensorfailuretypedescribedinSection3.3.Theprimary,secondary,andRPVparameterchangeeffects,ifany,werethenidentified,analyzed,andtabulated.Note:Becausesignalsfromthesecommonlinesandsensorswerefre-quentlyutilizedbycomponentspreviouslydeletedaspartof.step3.1.2above,forcompleteness,theseandquestionable,non-safetyorcontrolcomponentswereretainedintheanalysisgroups.12"25344
