Mr. Grimes SWE credentials were verified by review of Vermont Yankee's Phase 1 submittal report VY-RPT-12-00019 Rev.0 (Asset Suite EC40647) Pages 9 and H8.
Mr. Grimes' current Entergy qualification as Seismic Walkdown Engineer was also verified via the Entergy Plateau qualification system as part of the pre-job brief for this walkdown.

The design in calculation C15.0.3059 specifies 3/16" fillet welds, which also exists at the connection, therefore, the bolts are extra and the configuration is acceptable.

WO 349326 to remove blocks scheduled for 516/13 prior to restart. As of May 2 8th, this activity has been completed as confirmed by site personnel.

The SFP inlet line siphon breakers were specifically viewed and photographed as part of this walk-by. They were found to be in place with no obvious seismic interaction concerns.
