L-97-322AttachmentP~ae3VIolation810CFR50,AppendixB,CriterionV,Instructions,ProceduresandDrawings,requires,inpart,thatactivitiesaffectingqualityshallbeprescribedbydocumentedproceduresofatypeappropriatetothecircumstancesandshallbeaccomplishedinaccordancewiththoseprocedures.MaintenanceProcedure(MP)2-0950173,Revision9,"TheOverhaulofContainmentFanCoolersMotors2HVS-lA,1B,1C,1D(EQMotors),"Section9.9,implementsthepostmaintenancechecksofthecontainmentfancoolermotorsaftermaintenance.MP2-0950173requiresthefantobestartedinslowandfastspeedtoverifyproperbladerotationineachspeed.Contrarytotheabove,onMay16,1997,thelicenseefailedtoperformSection9.9ofMaintenanceProcedure2-0950173properlyinthatcontainmentfancooler2-HVS-1Dwasnotstartedinslowspeedandproperbladerotationwasnotverified.~Reaone1.FPLconcurswiththeviolation.REASONFORVIOLATIONThecausefortheviolationwaspersonnelerrorthatoccurredduringtheexecutionofpostmaintenancetestingperformedduringthelastUnit2refuelingoutage.Thiserrorresultedinreverserotationofcontainmentfancooler2-HVS-1D.TherootcauseevaluationconcludedthattheimpropertestexecutiononMay16,1997,wascausedbyfaultycommunicationsbetweenthemaintenancepersonnelassessingfanrotationinsidecontainmentandthecontrolroomoperatorwhostartedthefan.FPLreviewedthecompleteddocumentationfortheMay16,1997,postmaintenancetestandcompareditagainsttheMay16,1997,Unit2controlroomsequenceofeventsrecorder(SER).TheSERshowedthat2-HVS-1Dwasstartedtwiceduringthepostmaintenancetesting;however,boththeslowandfastspeedrotationalcheckswereperformedwiththemotoroperatinginfastspeed.Thefactthattwomotorstartsweredoneimpliesthatmaintenancepersonnelwerecorrectlyfollowingthetestproceduresteps.Theproperuseofthreewaycommunications,asrequiredbyboththeConductofMaintenanceandConductofOperationspolicies,wouldhavepreventedthefaultycommunicationwhichledtotheunsatisfactoryexecutionofthepostmaintenancetes L-97-322AttachmentP~ae43.CORRECTIVESTEPSTAKENANDTHERESULTSACHIEVEDA.ProcedureMP-2-0950173,"TheOverhaulofContainmentFanCoolerMotors2-HVS-lA,1B,1C,1D(EQMotors),"wasrevisedtorequirethefollowing:1)verificationsignaturesforbothfastandslowspeedmotorrotationtesting;2)acautionstatementtoensurethatbothwindingsareenergizedtoverifypropermotorrotationsubsequenttoelectricallyreconnectingthemotor;3)anaddendumsheetthatwillbeusedbythecontrolroomoperatorstofollowtheperformanceofthemotorrotationchecks;and,4)arequirementtoperformafunctionaltestofthecontainmentfancoolerinbothfastandslowspeedpriortoreturningtheunittoservice(i.e.,nolowflowalarmspresentduringrotationchecks).TheprocedurerevisionswerecompletedonNovember28,1997.AdditionaleditorialclarificationsweresubsequentlyaddedandapprovedonDecember17,1997.4.CORRECTIVESTEPSTOAVOIDFURTHERVIOLATIONSA.Thisspecificevent,includingtheimportanceofaccuratecommunicationsandtheuseofthreewaycommunications,wasincludedinaMaintenancetrainingbulletinthatwasissuedonNovember22,1997.B.ProcedureADM-78.01,"PostMaintenanceTesting,"willberevisedtoaddacautionnoteabouttestingmultiplespeedormultiplewindingmotorsbyJanuary31,1998.5.FullcompliancewasachievedonNovember28,1997,whentheprocedurerevisionstoMP-2-0950173,"TheOverhaulofContainmentFanCoolerMotors2-HVS-1A,1B,1C,1D(EQMotors),"werecomplete
