Response

Corrective Action Taken and Results Achieved:

The piping contractor, Pullman-Higgins, has revised the expansion anchor process sheet in order to make applicability of the witness and hold points clear to craft and inspection personnel. Witness points shall be verified by the QC inspector at any point in the installation process, while hold points require QC approval prior to proceeding to the next operation.

Unacceptable initial torque shall be cause for rejection and will require a new process sheet in order to retorque the installation.

Pullman-Higgins issued Field Instruction 177 to explain the use of the new process sheets and to effect their use.

Previously released field erection packages which contain the obsolete form will be returned to the hanger department for issuance of the revised process sheet.

Corrective action was completed on November 10, 1982.
