SIXTH REFUELING INSERVICE INSPECTION

SUMMARY

REPORT 1.0 Introduction /NIS 1 Form l.1 Reporting Requirements ASME Section XI,1980 Edition thru Winter 81 Addenda, IWA 6000, requires that a summary report be issued upon completion of.

inspections conducted during a refueling outage.

The selection of items examined during.this refueling outage was based upon the requirements of the Waterford 3 Ten Year Inservice

-Inspection (ISI) Program.

Refuel 6 was the second refueling outage'of the third inspection period in the first ten year _ interval.

1.2 Summary Report Coverage i

This summary report covers Class 1, 2 and-3 components and-their supports:(listed by code item number in sections 2.2, 3.2^'4.2 and 5.2) that were examined between the start up Reactor Coolant System (RCS) leakage tests of the fifth and-sixth refueling outages (11/6/92 to 4/19/94) at the Waterford 3 (W3) Steam Electric Station (SES), Hwy 18, Killona, LA.

This report includes summaries of examinations, abstracts of conditions found with corrective actions taken, Form NIS 1 "0wner's Data Report for Inservice Inspections" and a set of Form NIS-2

" Owner's Report for Repairs or Replacements." Although not ' required by the currently applicable Section XI Code and Addenda, these NIS-2 forms document Class 1 and 2 components and their supports repaired and/or replaced during this-same time period.

Items examined during this outage with limitations are identified and will be included in the applicable relief request in the next revision _of the W3 Ten Year ISI Program.

Steam Generator tube eddy current (ET) examinations during this outage were performed in accordance with W3 Technical Specifications and are not included in this_ report.

h SIXTH REFUELING INSERVICE INSPECTION

SUMMARY

REPORT 1.3 Implementation, Standards. Test Equipment and Records Entergy personnel developed, and the Factory Mutual Authorized Nuclear Insurance Inspector (ANII) reviewed, the procedures for the five methods of NDE used for ISI during this outage: magnetic particle (MT), liquid penetrant (PT). radiography (RT). ultrasonic i

(UT) and visual (VT).

Entergy also provided the supervisory control of contract Non-Destructive Examination (NDE) personnel.

l Calibration standards used during this outage were fabricated to j

conform to Section XI Code requirements.

l Test equipment included Sonic 1360,' USK 7 and USK 7D instruments' for l

UT examinations with transducers by Aerotech, Megasonics, RTD and Sigma. Additional. and more detailed, information is available on individual NDE reports.

An engineering evaluation determined that the component support could perform its intended design function.

The bearings were reworked. The VT-3 retest was satisfactory.

An engineering evaluation determined that the component support could perform its intended design function. The bearings were reworked.

CI 290317/WA 99000433 2.3.3 Component SI-142A is the LPSI header to RC loop 2B inside containment 8" check valve.

A visual examination revealed boron accumulation on both hinge covers. The boron accumulation was removed. The VT-1 retest was satisfactory.

CI 290299/WA 99000433 2.3.4 Component SI-143A is the LPSI header to RC loop 2A inside containment 8" check valve.

A visual examination revealed boron accumulation on both hinge covers and body to bonnet bolting.

Boron accumulation was removed.

The VT-1 retest was satisfactory.

CI 290298/WA 99000433 2.3.5 Component SI-512B is the RC loop 2 hot leg injection 3" check valve. A visual examination revealed boron accumulation on the valve body and bolting.

Boron accumulation was removed.

The VT-1 retest was satisfactory.

CI 289902/WA 01121949 2.3.6 Component support SIRR-1101.

A VT-3 examination revealed the clamp and bolting were severely corroded. An engineering evaluation determined that the component support could perform its intended design function.

The clamp was replaced. The VT-3 l

retest was satisfactory.

3.3.1 Continued i-As recommended by IN 93-20, an enhanced examination technique was used which increased the sensitivity above ASME Section XI requirements.

This examination revealed 8 inner diameter -(ID) connected flaws ranging in length from 1/2" to 2-1/2" and estimated to be s 0.050" through-wall depth. Although these indications did not exceed pre-outage determined allowances and were well within the acceptance criteria of ASME Section XI, they will be monitored in future refueling outages to comply with the recommendation of in-progress engine dng analysis, PEIR DE-59.

3.3.2 Component support CSRR-0191.

VT-3 examination revealed a' loose turnbuckle lower nut. NOC tightened the nut. VT-3 retest was satisfactory.

An engineering evaluation determined _ that the condition found was acceptable and that no field work was required.

Snubbers SISR-921 and SISR-1044 required testing in accordance with plant programs.

Normal scheduled time for testing is during refueling outages since systems where snubbers are installed are normally not operable.

However, safety injection system these snubbers are installed on are required to be operable during refueling outages.

Therefore, these snubbers were removed and replaced with snubbers obtained from stores.

Replacement snubbers were tested in accordance with plant programs prior to installation.

Suubbers removed were tested in accordance with plant programs.

Removed snubbers will be returned to stores to be installed when necessary.

Material certification for replacement snubbers is filed as follows:

Refueling water storage pool outlet header "B" check valve (SI MVAAA107 B) exhibited boric acid build-up due to leakage.

During rework to replace installation gaskets engineering evaluation determined installation bolts did not provide sufficient strenght to achieve proper gasket-sealing.

Therefore, bolting was replaced with higher strength material obtained from stores.

During refueling outage six both pressurizer safety valves were removed and replaced with spare valves.

Replacement valves were reworked and tested with satisfactory results prior to installation.

Removed valves will be tested and reworked as necessary.

Following installation and return to service replacement valves were satisfactorily pressure tested to System Leakage Test criteria.

Rework and testing of replacement valves was performed under Contract Number W-1390-0010 and Work Authorizations 01114994 and 01115003.

Work Authorization 01113436 performed eddy current. testing of Steam Generator No.l.

This testing required removal of steam generator manways.

During inspection of steam generator manway studs two studs were found to have damaged threads.

Therefore, two steam generator manway studs were replaced.

Preparation of repair / replacement plan did not occur until after installation of replacement studs.

This is documented on Condition Report CR,

416.

Eddy current inspection of' steam generator no. 1 and 2 tubes showed several tubes that required plugging.

Plugging of tubes l

was performed on WA 01119373 for Steam Generator No. 1 and WA i

01119380 for Steam Generator No. 2.

Plugs were rolled plugs.

A previously installed mechanical tube plug on hot leg row 121 line 45 of steam generator no. I was found leaking.

This plug was replaced with a welded plug on WA 01122133.

This welded plug was installed by B&W Nuclear Service Company under a Waterford 3 repair / replacement plan.

Preparation or repair / replacement plan was performed per Section XI 1980 Edition through Winter 1981 addenda.

Repair / replacement plan was. prepared for installation of plug using a procedure qualified by B&W to Section XI 1986 Edition through 1989 Edition which was reviewed and approved by Waterford 3.

Engineering determined leakage from.

hinge pin covers occurs due to low strength fasteners not providing sufficient strength to adequately crush gasket and achieve a seal.

Work Authorizations (WA) 01122332, 01122320 and 01122336 respectively provided work instructions

, to replace hinge pin cover bolting with higher strength bolting.

This change was addressed in SPEER 9401232.

This SPEER also addressed using Class 2 bolting in lieu of Class 1 bolting in this application due to identical requirements for Class 1 and Class 2 material in this size range.

Condition Identification (CI) 290913 documents seat leakage of Safety Injection Header IB Check Valve (SI MVAAA335 B).

Work Authorization 01123031 provided work instructions to rework valve.

During valve rework one hinge pin' cover nut was lost and required replacement.

Hinge pin cover nut was replace with an identical item obtained from stores.

Following return to service all valves were satisfactorily pressure tested to System Leakage Pressure Test criteria.

Functional testing of Main Steam Safety Valves is required every third refueling cutage.

Functional testing of Main Steam Safety Valves had.been performed as scheduled.

However, disassembly of Main Steam Safety Valves for preventive maintenance had not been performed since start-up.

Therefore, replacement of three Main Steam Safety Valves with three valves from warehouse spares was performed to minimize outage impact.

The three main steam safety valves _ removed will be functional tested and reworked on separate work packages.

Following installation of spare Main Steam Safety Valves the valves were successfully pressure tested using System Inservice Pressure Testing criteria.

. Main Steam Safety Valves MS MVAAA106 B, MS MVAAA108 B and MS MVAAA110 B required periodic set pressure testing.

Set pressure testing was performed and documented on WA's 01116802, 01116800 and 01116795 respectively.

I addition to set pressure testing these valves were disassembled for inspection and preventive maintenance.

Inspection showed steam cutting of discs and nozzles on all three valves.

Therefore, discs and nozzles were replaced on.all three valves.

Following installation discs and nozzles valves were set pressure.

Condition Identification documented leakage of Letdown Heat Exchanger Outlet Header Relief Valve (CVCMVAAA12 6).

Work Authorization (WA) 01092483 work instructions called for rework of spare valve serial number N59357-00-0001 followed by installation of this spare valve.

This spare valve was removed on WA 01030693 due to leakage.

Rework inspection showed cutting of nozz.e seating surfaces due to seat leakage.

Machining to remove 0.005" was required to achieve a satisfactory seating surface.

A dye penetrant (PT) examination of machined surface was performed following machining with satisfactory results.

Disc seating surface was satisfactory following lapping.

Following reassembly valve was bench tested to required set pressure and leakage requriements.

Additional observation showed that valve serial number N59357-00-0002 currently installed in system was not leaking.

Therefore, valve replacement was not performed.

Performance of system functional or inservice pressure testing will occurr following installation of valve on system.

Condition Identification (CI) 283412 documented lifting of Reactor Coolant Loop 1 Hot Leg Injection Relief to Waste Tanks (SI MVAAA503 A).

Work Authorization (WA) 01102519 provided work instructions for rework of valve.

During rework valve disc was found to have cutting due to seat leakage and spring washers were found to have galling due to valve chatter.

Therefore, disc, spring and spring washers were replaced with identical items from i

stor.es.

Following reassembly valve bench tested to required set

pressure and leakage requirements.

Following installation on system valve was satisfactorily pressure tested to system functional pressure test requirements.

Repetitive Task (RT) 001504 requires performance of sampling and functional testing of Type 1 Small Mechanical Snubbers.

Work Authorization (WA) 01112395 provided work instructions to perform sampling and functional testing.

Snubber SISR-1067 failed functional testing.

Due to failure of functional testing snubber SISR-1067 was replaced with a like item obtained from stores.

Condition Identification (CI) 289186 and 289187 document a requirement to perform functional testing of snubbers SISR-707 and SISR-737 respectively during Refueling Outage Six.

Work Authorization (WA) 01119697 and 01119698 respectively provided work instructions to perform this functional testing.

Both SISR-707 and SISR-737 failed functional testing.

Therefore, both saubbers were replaced with like items obtained from stores.

All replacement snubbers were satisfactorily tested to PE-005-014

" Snubber Sampling and Functional Testing" prior to installation.

Following installation all snubbers were satisfactorily inspected to VT-3 and PE-005-011 " Mechanical Snubber Visual Inspection" requirements.

opening Containment Spray Header A Isolation Valve (CS MVAAA125 A).

Work Authorization (WA) 01113728 provides work instructions to rework this valve.

During rework valve gate surfaces were found scoured.

Scouring was significant enough to prevent obtaining an effective seal and restrict closure.

Therefore, valve gate was replaced with an identical item obtained from stores.

Replacement gate was obtained from original valve manufacturer W-K-M who is now owned by Cooper Industries.

W-K-M is no longer a Code certified supplier.

Therefore, replacement l

gate was obtained using guidance provided in NRC Generic Letter l

89-09.

SPEER 9301165 was prepared to provide configuration l

control and justification for obtaining and installing this l

replacement gate from Cooper.

l Condition Identification (CI) 288831 documents seat leakage of Containment Spray Header B Isolation Valve'(CS MVAAAl25 B).

Work Authorization (WA) 01119131 provides work instructions for performing corrective maintenance to correct seat leakage.

During performance of corrective maintenance valve gate was found to have scouring of sealing surfaces.

Scouring of sealing i

surfaces was of sufficient nature replacement was required.

l Valve gate was replaced with an identical item obtained from stores.

l Both valves had stems and other Non-Code. items replaced.

Following assembly both valves were satisfactorily pressure tested to system functional pressure test' requirements.

Work Authorization (WA) 01122196 provided work instructions for replacement of a gasket on flange.

Upon disassembly flange faces were found misaligned.

Flange was actually on line 2CH2-165A/B.

To correct flange misalignment one half of weld on one flange was ground and rewelded to pull flange straight.

This corrected flange misalignment. A new gasket was installed and flanges bolted together.

Following return to service flange and line_ were satisfactorily pressure tested to System Functional Pressure Test criteria.-

l Original weld Specification Documentation Sheet (WSDS) was lost along with original Section XI Repair / Replacement and Suitability Evaluation Form.

A Condition Report has been initiated to document this error and provide corrective action.

A review of QA logs shows all inspections were performed.

l In addition NDE reports are available for weld Dye Penetrant Exam (PT) and VT-l 2 exam for System Functional Pressure Test.

Weld filler material control form was still in work package. This provided documentation on welder and weld filler material used. Therefore, all inspections and documentation normally provided on a WSDS is available.

During rework discs and. seats in.both valves were found to have scouring of sealing surfaces. Therefore; discs and seats.were replaced with identical. items obtained from stores.

Seats are not considered ASME-Code-Pressure Retaining items.

Use of Air Operated Valve diagnostics'was used.to attempt to set valves properly and avoid future disc.and seat problems.

Following return to_ service both valves.were satisfactorily pressureLtested to System Functional Pressure Test-criteria.
